Paraganglioma treatment with subtotal petrosectomy. Case report

Summary: Paragangliomas are rare tumors, particularly in head and neck region. They arise from cells of the autonomic nervous system and are more frequently found in the carotid body, jugular body, glossopharyngeal nerve and vagus nerve. The identification of this neoplasm in the middle ear is based on the clinical manifestations and complementary exams (audiogram, tympanogram, computed tomography and magnetic resonance). The purpose of this case report is to demonstrate that subtotal petrosectomy is a valuable surgical technique for the treatment of glomus tumors of the temporal bone. A 45-year old woman was referred to our hospital for the surgical treatment of a jugulotympanic paraganglioma on the  left ear. The operation included a subtotal petrosectomy, and the complete removal of the tumor was achieved. Subtotal petrosectomy can be performed as an isolated surgical procedure, but most commonly the technique is used as the first surgical step of different approaches for the lateral skull base. Subtotal petrosectomy includes: 1) the emptying of the temporal bone content with preservation of the profound cortical bone and of the labyrinth; 2) the permanent closure of the external auditory canal; 3) the closure of the tympanic pharyngeal orifice of the Eustachian tube; and 4) the obliteration of the cavity with autologous abdominal fat. In the case reported, the patient was discharged at the ninth day after surgery. As surgical consequence, inherent to the technique, the patient had definitive conductive hearing loss. Follow-up was performed annually with magnetic resonance. The therapeutic options for the paragangliomas of the temporal bone include surveillance without therapy, radiotherapy and surgery. The only curative solution is the surgery. In our patient we have opted for the surgical treatment which included the subtotal petrosectomy. One year after surgery, the patient is healed, having returned to her normal activity.

Desenvolvem-se a partir das células do sistema nervoso autónomo, sendo mais frequentemente encontrados no corpo carotídeo, no corpo jugular, ao longo do nervo glossofaríngeo e no nervo vago. A identificação deste tipo de lesões no ouvido médio baseia-se na anamnese, na observação e em exames complementares de diagnóstico como o audiograma, a imitância acústica (impedanciometria), a tomografia computorizada e a ressonância magnética. O objectivo deste trabalho é o de demonstrar a utilidade da petrosectomia subtotal como uma técnica, ou tempo cirúrgico, que está incluída nas abordagens realizadas para o tratamento dos paragangliomas do osso temporal. Um doente de 45 anos de idade foi referenciado para o Serviço de Otorrinolaringologia do Hospital de Egas Moniz com o diagnóstico de paraganglioma jugulo-timpânico à direita, para tratamento cirúrgico. A operação incluiu a petrosectomia subtotal, tendo obtido a excisão completa do tumor. A petrosectomia subtotal é utilizada nos acessos laterais à base do crânio nos quais seja necessário remover toda a porção pneumatizada do osso temporal. Pode ser realizada como um procedimento isolado ou, mais frequentemente, é o tempo cirúrgico inicial de uma abordagem que se estende a outra área anatómica vizinha do osso temporal. A petrosectomia subtotal consiste na eliminação de todas os grupos celulares do osso temporal com preservação das corticais ósseas profundas e da cápsula ótica. A operação implica o encerramento definitivo do canal auditivo externo e do orifício timpânico da trompa de Eustáquio, assim como a obliteração da cavidade com gordura abdominal autóloga. No caso clínico em concreto a petrosectomia subtotal, estendida à região infralabiríntica e cervical (via da Fossa Infratemporal do tipo A) foi utilizada para remover a lesão do osso temporal. O doente teve alta ao nono dia de pós-operatório, apresentando com sequela cirúrgica uma hipoacusia de condução permanente. O follow-up foi realizado periodicamente com recurso a exames imagiológicos, nomeadamente a ressonância magnética. As opções terapêuticas possíveis para os paragangliomas são várias, desde a vigilância sem tratamento, à radioterapia e à cirurgia. No entanto, a única terapêutica curativa é a cirúrgica. No nosso doente optámos pela realização de uma via de abordagem que incluía a petrosectomia subtotal.
