Prescription of antibiotics in after-hours clinics

Summary: Aim: To assess antibiotic prescribing in an out-of-hours attendance service. Design of study: Cross-sectional, descriptive, observational survey. Setting: A primary health care facility (Vialonga Health Unit, Póvoa de Stª Iria Health Centre). Methods: A random sample of medical episode records of out-of-hours attendance service from the year 2000 was performed. 1722 records were studied. Variables studied were the patients’ gender, age, antibiotic prescribing, drug used and pharmacotherapeutic group, diagnosis and doctor prescribing. Results and conclusions: There was registration of antibiotic prescription in 399 (23.3%) of the episode records. Macrolides accounted for the group more frequently prescribed (30% of prescriptions) and benzylpenicillins were chosen in 2,2 % of cases. Acute tonsillitis accounted for the majority of antibiotic prescriptions. Taking into account the growing resistance of group A streptococci to macrolides, these results deserve attention. Third generation cephalosporins and quinolones were widely used, in 10.2% and 7.7% of cases respectively. In 19 cases of acute otitis media (29,7%) third generation cephalosporins, not recommended by available evidence, were used. Quinolones were the drugs of choice in 67% of cystitis cases, and cotrimoxazole, the drug of first choice for uncomplicated cystitis according to current recommendations, was used in 7.5%. The acknowledgement and compliance with current guidelines for antibiotic use is an urgent task.

Tipo de estudo: transversal, descritivo, observacional. Local : Unidade de Saúde de Vialonga, extensão do Centro de Saúde da Póvoa de Sta Iria. Metodologia: Estudaram-se fichas de registo médico referentes às consultas no atendimento complementar, no ano 2000. Utilizou-se uma amostra aleatória em etapas sucessivas com a dimensão de 1722 fichas. Mediram-se as variáveis: sexo, idade, prescrição de antibiótico, princípio activo e grupo farmacoterapêutico, diagnóstico no caso de prescrição de antibacteriano e médico prescritor. Resultados e Conclusões: Ocorreu registo de prescrição de antibióticos em 399 fichas (23,3% das consultas). O grupo mais prescrito foi o dos macrólidos (30% dos casos), tendo as benzilpenicilinas sido escolhidas em 2,2% dos casos. A amigdalite aguda foi o diagnóstico face ao qual houve maior prescrição de antibióticos. Perante a evidência da crescente resistência dos estreptococos do grupo A aos macrólidos estes resultados merecem reflexão. As cefalosporinas de 3ª geração e as quinolonas foram dos grupos mais usados, em 10,2% e 7,7% dos casos, respectivamente. As suas escassas indicações em Medicina Geral e Familiar questionam tais valores. Em 19 otites (29,7%) foram prescritas cefalosporinas de 3ª geração. A evidência não o recomenda. As quinolonas foram a terapêutica de eleição em 67% das cistites; o cotrimoxazol em 7,5%. As recomendações actuais indicam-no como primeira escolha nas cistites não complicadas.
