Infectious Endocarditis: A year of changing patterns

Resumo: The authors made a retrospective analysis of 15 case reports of infectious endocarditis diagnosed in 2004, which represented a significant increase in comparison to the four previous years. The objective was to look for epidemiological and clinical characteristics that could justify this increase. In 2004 the mean age increased about a decade and the predisposing factors were inverted. Valvular heart disease has assumed relevance and there was a decrease of about 50% in drug abuse and HIV infection among patients with endocarditis. The sub-acute clinical presentation almost doubled and fever was the only manifestation that remained unchanged during these years. The pathogens isolated in blood cultures were different with an important reduction in MRSA. There was an increase in embolic cerebral complications and valvular complications with urgent need for surgery but significantly less mortality. Conclusion: in 2004 there was a signifi cant increase in infectious endocarditis with a change in the epidemiological, clinical and microbiological patterns. A causal factor that could explain all these changes was not identified.

A causal factor that could explain all these changes was not identified.Os autores analisaram retrospectivamente 15 casos de endocardite infecciosa diagnosticados no ano de 2004, o que representou um aumento significativo de casos em relação aos quatro anos anteriores, com os quais foram comparados. O objectivo foi procurar características epidemiológicas e clínicas que pudessem justificar este aumento. Verifi caram que em 2004 a média de idades aumentou em cerca de uma década e os factores predisponentes inverteram-se. A patologia valvular assumiu preponderância e registou-se um decréscimo de mais de 50% na toxicodependência e infecção VIH nos doentes com endocardite. A apresentação clínica subaguda quase duplicou, sendo a febre a única manifestação que se manteve invariável ao longo dos anos. Foram isolados nas hemoculturas agentes diferentes do que era habitual, verificando-se uma redução importante no SAMS. Houve um aumento de complicações embólicas cerebrais e complicações valvulares com necessidade de cirurgia urgente, mas a mortalidade foi significativamente menor. Conclusão: em 2004 registou-se um aumento significativo de endocardites infecciosas acompanhado de uma mudança nos seus padrões epidemiológicos, clínicos e microbiológicos.
