Summary: Von Recklinghausen neurofibromatosis (neuro­fibromatosis type 1) is an autosomal dominant disorder that occurs in 1 in 3,000 life births. Ma­lignant degeneration of subcutaneous neurofibro­ma occurs in about 5 to 10 percent of patients; however, manifestations in the vagus nerve are rare. There are five reported cases of malignant schwan­noma of the intrathoracic vagus nerve, and only one was associated with von Recklinghausen neurofi­bromatosis. We believe that we present the second reported case of a malignant peripheral nerve she­ath tumor, involving the vagus nerve in a patient with von Recklinghausen neurofibromatosis.

Estes doentes estão expostos a ris­co acrescido no desenvolvimento de tumores a partir das células da bainha dos nervos periféri­cos, nomeadamente das células de Schwann, sen­ do designados schwannomas. Estes tumores ex­cepcionalmente envolvem o nervo vago intratorá­cico e raramente assumem as caracteristicas de malignidade. . Os autores apresentam um caso de uma doente portadora de neurofibromatose tipo 1, na qual a TAC torácica revelou uma massa bem definida localizada nos quadrantes superiores do medi­astino, com 10,6 cm de maior diâmetro. Foi sub­metida a toracotomia com exérese de tumor em relação com o nervo vago esquerdo. O diagnósti­co histológico foi schwannoma maligno do vago.
