Facilitators to physical activity in people with interstitial lung disease: a qualitative study

Summary: Background: Interstitial lung disease (ILD) is characterized by a group of heterogeneous and progressive respiratory diseases. The main symptoms are persistent dry cough, dyspnoea, fatigue, muscle and joint pain. These symptoms lead to a decrease in physical activity levels in this population, which contributes to accelerating the progression of the disease. Knowledge about the facilitators of physical activity in people with ILD can contribute to the creation of interventions and programs that are better adapted to their needs and routines, ultimately reducing the progression of the disease. Objective: This study aimed to understand the facilitators of physical activity (PA) at home in people with ILD. Methods: A qualitative study was carried out using semi-structured individual interviews with people with ILD in the stable phase of the disease. The interviews were recorded, manually transcribed in full and analyzed using thematic analysis. Results: 11 participants were interviewed (76±10 years; 6 [55%] women; FVC%p 75.5±14.2; DLCO%p 43.2±20). Two main themes emerged from this study: physical activity and expectations and thoughts about iLiFE. The first was described in 3 sub-themes: self-perception, daily activities and impacts. The second was described in 6 sub-themes: getting better and living longer; thirst for knowledge; practical physical support; design; contribution to the future and willingness to improve. Conclusion: Given the importance of greater adherence and, above all, regularity in the practice of physical activity among people with ILD, these results highlight the need to deepen knowledge about the factors that encourage and promote practice in this population. Such an understanding will enable the development of more personalized and effective plans and actions as early as possible.

Os principais sintomas são tosse seca persistente, dispneia, fadiga, dores musculares e articulares. Estes sintomas levam a uma diminuição dos níveis de atividade física nesta população, o que contribui para acelerar a progressão da doença. O conhecimento sobre os facilitadores para a prática de atividade física em pessoas com DPI pode contribuir para a criação de intervenções e programas mais adaptados às necessidades e rotinas reduzindo, em última instância, a progressão da doença. Objetivo: Este estudo teve como objetivo conhecer os facilitadores à prática de atividade física (AF) em casa em pessoas com DPI. Métodos: Realizou-se um estudo qualitativo, que utilizou entrevistas individuais semiestruturadas de pessoas com DPI, em fase estável da doença. As entrevistas foram gravadas, transcritas manualmente na íntegra e analisadas com análise temática. Resultados: Foram entrevistados 11 participantes (76±10 anos; 6 [55%] mulheres; CVF%p 75,5±14,2; DLCO%p 43,2±20). Dois temas principais emergiram desse estudo: atividade física e expectativas e pensamentos sobre o iLiFE. O primeiro, foi descrito em 3 subtemas: autoperceção, atividades diárias e impactos. O segundo, foi descrito em 6 subtemas: melhorar e viver mais; sede de conhecimento; suporte físico prático; design; contribuição para o futuro e vontade de melhorar. Conclusão: Devido à importância de uma maior adesão e, sobretudo, uma regularidade na prática de atividade física entre pessoas com DPI, estes resultados evidenciam a necessidade de aprofundar o conhecimento sobre os fatores que incentivam e promovem a prática nessa população.
