It is composed by high specialized microvessels, surrounded by astrocytes, pericytes, neurons and microglia cells, which tightly control the influx and efflux of substances to the brain parenchyma. During aging, the BBB becomes impaired, and it may contribute to the development of neurodegenerative and neurological disorders including Alzheimer’s disease and other dementias. Restoring BBB can be a strategy to prevent disease onset/development and reduce the symptoms of these conditions. This work reviews the principal mechanisms associated with BBB breakdown in healthy and unhealthy aging, as well as biomarkers and methodologies that assess its impairment. Complementarily, potential therapeutic targets for restoring the normal function of the BBB in ageing are herein discussed.A barreira hemato-encefálica (BHE) é uma estrutura indispensável para a manutenção do microambiente do sistema nervoso central (SNC), necessário para um adequado funcionamento neuronal. É composta por microvasos altamente especializados, rodeados por astrócitos, pericitos, neurónios e células da microglia, cuja interação dinâmica controla rigorosamente o influxo e efluxo de substâncias para o parênquima cerebral.No decorrer do envelhecimento, a BHE sofre alterações celulares e bioquímicas, que podem comprometer a sua função e contribuir para o desenvolvimento de doenças neurodegenerativas e neurológicas, como a doença de Alzheimer e outras demências. Restaurar a estrutura e funcionamento da BHE pode ser uma estratégia para prevenir o aparecimento/ progressão de doenças e reduzir os sintomas destas condições patológicas. Este trabalho faz uma revisão crítica dos principais mecanismos associados à disfunção da BHE durante o envelhecimento saudável e não saudável, bem como dos biomarcadores e metodologias que avaliam essa disfunção.
