O Nurses' Role in Medication Reconciliation in Chronically Ill Patients in Ambulatory Care: A Scoping Review Protocol

Summary: Introduction: Medication reconciliation being a process that reduces the risk of medication discrepancies, will enable better management of the chronic condition to improve the patient's quality of life and prevent complications. Objective: To map the existing scientific evidence on the role of nurses in medication reconciliation for chronically ill patients in ambulatory care. This knowledge is essential for a more effective and adequate understanding of this phenomenon and to increase the safety and quality of care provided by nurses using evidence-based practice as a reference. Methods: The methodology chosen was a scoping review as proposed by the Joanna Briggs Institute (JBI) and considering the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). To define the eligibility criteria, it was followed the Population, Concept and Context (PCC) reference framework. Based on that, this review will consider studies that address the role of the nurse in medication reconciliation in the chronically ill in ambulatory care. Study selection, data extraction and synthesis will be carried out by two independent reviewers. The review will consider all studies, published and gray literature, without temporal limitations, in any language. Results: The results will be presented in a descriptive way, using tables, considering the objective and focus of the scoping review. Conclusion: It is expected that the results obtained will provide an overview of the importance of the nurse's role in medication reconciliation in chronically ill patients in ambulatory care.

Este conocimiento es crucial para una comprensión más eficaz y adecuada de este fenómeno y para permitir aumentar la seguridad y calidad de los cuidados proporcionados por los enfermeros, teniendo como referente una práctica basada en la evidencia Métodos: Se optó por la metodología de scoping review según lo propuesto por el Joanna Briggs Institute (JBI) y teniendo en cuenta los Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). Para la definición de los criterios de elegibilidad, se siguió el marco de referencia Población, Concepto y Contexto (PCC). Basándose en esta estructura, esta revisión considerará estudios que aborden el papel del enfermero en la reconciliación de la medicación en el paciente crónico en contexto ambulatorio. La selección de estudios, extracción de datos y síntesis serán realizadas por dos revisores independientes. Se considerarán todos los estudios, publicados y literatura gris, sin limitación temporal y en cualquier idioma. Resultados: Los resultados se presentarán de forma descriptiva y con el uso de tablas, teniendo en cuenta el objetivo y el enfoque de la scoping review. Conclusión: Se espera que los resultados proporcionen una visión general de la importancia del papel del enfermero en la reconciliación de la medicación en el paciente crónico en contexto ambulatorio.Introdução: A reconciliação da medicação sendo um processo que diminui o risco de discrepâncias na medicação, possibilitará uma melhor gestão da condição crónica de forma a melhorar a qualidade de vida do doente e prevenir complicações. Objetivo: Mapear a evidência científica existente sobre o papel do enfermeiro na reconciliação da medicação no doente crónico em ambulatório. Este conhecimento é fulcral para uma compreensão mais eficaz e adequada deste fenómeno e para permitir aumentar a segurança e qualidade dos cuidados prestados pelos enfermeiros tendo como referencial uma prática baseada na evidência. Métodos: Optou-se pela metodologia da scoping review conforme proposto pelo Joanna Briggs Institute (JBI) e tendo em consideração os Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). Para a definição dos critérios de elegibilidade foi seguido o quadro de referência População, Conceito e Contexto (PCC). Baseando-se nessa estrutura, esta revisão considerará estudos que abordem o papel do enfermeiro na reconciliação da medicação no doente crónico em contexto de ambulatório. A seleção dos estudos, extração de dados e síntese serão realizadas por dois revisores independentes. Serão considerados todos os estudos, publicados e literatura cinzenta, sem limitação temporal, e em qualquer idioma. Resultados: Os resultados serão apresentados de forma descritiva, e com recurso a tabelas, tendo em conta o objetivo e o foco da scoping review.
