Peritonsillar abscess. Review of 89 patients

Summary: | Objectives: To review the c/inical and therapeutic aspects of thecases of peritonsillar abscess observed during the last 13 years atour inpatient department.Patients and methods: Retrospective, transversal study of casesmanaged at the Department of Otorhinolaryngology at the Complejo Hospitalario Universitario de Vigo" (University Hospital ofVigo) from 1994 to 2006. Data was assessed for the 89 patientsdiagnosed with peritonsillar abscess, confirmed by fine needle aspiration.Results: Age distribution of patients showed that 83.3% of themwere 20-40 years-old. Male/female ratio was 1.211. The averagetime from the beginning of symptoms was 7.8 days and the mostfrequent symptoms were: sare throat (92. 7 %), fever (51. 7%) and trismus (50. 6%). Antibiotics were administered to 75.3% of patients before admission. Leucocytosis was present in 75.3% of cases, and left-shift (band forms excess) in 66.3%. Staphylococcus aureus was found in 26. 7% of cultured samples. The abscess was drained in 58.4% of cases. Average length of stay at the hospital was 4.5 days and there were no deaths.Conclusions: Peritonsillar abscess is an unfrequent entity in ENTinpatient departments, since majority of cases are managed ambulatorily. Fine-needle aspiration, antibiotic therapy and analgesic constitute the gold-standard for diagnosis and management. |

Fine-needle aspiration, antibiotic therapy and analgesic constitute the gold-standard for diagnosis and management.Objetivos: El objetivo dei presente trabajo es revisar los aspectosclínicos y terapêuticos dei absceso periamigdalino en los últimos 13anos en nuestro servido de hospitalización.Pacientes y métodos: Se diseiió un estudio retrospectivo de cortetransversal que se desarrollo en el Servicio de Otorrinolaringologíadei Complejo Hospitalario Universitario de Vigo. lngresaron a esteestudio 89 pacientes con el diagnóstico de absceso periamigdalinoconfirmado mediante punción-aspiración, que fueron atendidos entre los anos 1994 y 2006.Resultados: El 83.3% de nuest ros pacientes se encontrá entre los 20 y 40 anos de edad; siendo la proporción hombre/mujer: 1.2/1.EI tiempo de enfermedad fue de 7.8 dias. Dentro de los sintomasmas frecuentes encontramos: dolor de garganta (92.1 %), fiebre(5 1. 7%) y trismus (50.6%). EI 75 .3% recibió antibióticos antes de ser hospitalizado. Hubo leucocitosis en 75.3% de los pacientes ydesviación izquierda en 66.3%. Staphylococcus aureus se encontroen el 26.7% de las secreciones cultivadas. Se hizo punción aspiración en todos los pacientes, drenándose el absceso en 58.4% de los casos. El t iempo de hospitalización fue de 4.5 dias. No ocurrieron desenlaces fatales.Conclusiones: Encontramos que se trata de una entidad relativamente poco frecuente en los servicios de hospitalización, siendo la mayoría de los pacientes manejados de forma ambulatoria.
