CO2 laser surgery for the treatment of glottic cancer

Resumo: Introduction: Transoral laser microsurgery (TLM) can beconsidered a valid alternative in the treatment of glotticcancer.Matherial and Methods: Retrospective study, considering allpatients treated with TLM, from January 2000 to December2005. Patients were considered for the present study if theyunderwent laser surgery as primary treatment of glotticcancer.Results: 45 patients: CIS (n=9), T1a (n=29), T1b (n=5), T2(n=1), T3 (n=1). The mean duration of follow-up was 63,3months. The rate of global local recurrence was 22,2%, withthe anterior commissure involvement being responsible for40% of the recurrences. The 5-year overall survival and the5-year disease-free survival were 82,2%, and the laryngealpreservation rate was 89,2%.Conclusion: This study supports the role of TLM in treatingearly-stage glottic cancer. The highest recurrence rates occurwith positive margins and anterior commissure involvement.Advanced glottic cancers are not a consensual indication forlaser surgery, usually with worse oncological outcome.

Obteve-se uma taxa de recidiva local global de 22,2%,verificando-se o envolvimento da comissura anterior em 40%das recidivas. A taxa de sobrevida global e livre de doença, aos5 anos, foi de 82,2%, com uma taxa de preservação laríngeade 89,2%.Conclusão: O laser CO2 é uma opção eficaz na abordagem deneoplasias glóticas em estadio inicial. A taxa de recidiva tumoralé maior quando há tumor na margem e/ou envolvimento dacomissura anterior.
