Does nasal surgery really matters? The importance of nasal surgery in tympanoplasty success

Resumo: The presence of concomitant nasal disease in the preoperative evaluation of chronic otitis media sugery raises some issues in terms of surgical decision. Empirically a transient tubal dysfunction can result from nasal surgery and it may be associated with failure of tymplanoplasty. This study aims to evaluate the success of tympanoplasty in patients receiving both nasal and tympanomastoidectomy surgery. Retrospective analysis of 6 patients who had undergone simultaneous nasal surgery and tympanoplastic surgery. The overall success rate of tympanic graft and aeration of the middle ear was 83.3%. All patients presented a decrease of the air-bone gap (ABG) with tympanoplasty. The average ABG postoperatively was 23.2 dB. The findings are consistent with the rates described in other studies for tympanoplasty without simultaneous nasal surgery. Our results suggest that concomitant nasal surgery do not impairs the success of tympanoplasty.

Empiricamente pode presumir-se que uma disfunção tubária transitória possa resultar da cirurgia nasal e estar associada a falência da timpanoplastia. Este trabalho pretende avaliar o sucesso da timpanoplastia em doentes submetidos simultaneamente a cirurgia nasal e timpanomastoidectomia. Foi realizada a análise retrospectiva de 6 doentes submetidos simultaneamente a cirurgia nasal e timpanomastoidectomia. Obteve-se uma taxa de sucesso global de enxerto timpânico e de arejamento do ouvido médio de 83.3%. Todos os doentes avaliados apresentaram diminuição do gap aéreo-ósseo (GAO), tendo a média de GAO no pós-operatório sido de 23.2 dB. Os achados são compatíveis com as taxas descritas noutros estudos para timpanoplastia sem cirurgia nasal simultânea.
