USE OF APICECTOMY IN PARENDODONTIC SURGERY: A narrative literature review

Resumo: | Parendodontic surgery is a procedure used in chronic cases of persistent periapical infection, in addition to cases of calcified root canals where conventional endodontic treatment has not been successful. This technique may be associated with apicoectomy for cases in which it is necessary to apicotomize a root, and is considered a very invasive procedure. Even so, it is a modality that receives great prominence among endodontic procedures, since it aims to remove bacteria present in the apical region, in addition to preventing other microorganisms from re-entering the apical tissues of the canal. Given the importance of this therapeutic technique, the present study aimed to discuss the impressions contained in the literature regarding the use of apicoectomy in parendodontic surgery, discussing its indications and importance in current clinical practice. The study was developed through a narrative review of the literature, with the selection of studies published between 2020 and 2024 and made available in the SCIELO, Publimed and Google Scholar databases. Based on the application of the research criteria, eight studies were selected, which reinforce the importance of endodontic surgery associated with apicoectomy as a highly efficient technique for cases of failure of conventional endodontic treatment. Through the selected studies, it was found that when indicated appropriately and executed correctly by professionals, this technique allows for efficient treatment. In addition, it is essential that professionals who perform this technique have the necessary knowledge and skills, in addition to the use of appropriate materials that can guarantee the best therapeutic results. |

Esta técnica puede asociarse a la apicectomía para los casos en los que es necesario apicotomizar una raíz, y se considera un procedimiento muy invasivo. Aun así, es una modalidad que recibe gran protagonismo entre los procedimientos endodónticos, ya que tiene como objetivo eliminar las bacterias presentes en la región apical, además de evitar que otros microorganismos vuelvan a entrar en los tejidos apicales del conducto. Dada la importancia de esta técnica terapéutica, el presente estudio tuvo como objetivo discutir las impresiones contenidas en la literatura sobre el uso de la apicectomía en la cirugía parendodóntica, discutiendo sus indicaciones e importancia en la práctica clínica actual. El estudio se desarrolló a través de una revisión narrativa de la literatura, con la selección de estudios publicados entre 2020 y 2024 y puestos a disposición en las bases de datos SCIELO, Publimed y Google Scholar. Con base en la aplicación de los criterios de investigación, se seleccionaron ocho estudios, los cuales refuerzan la importancia de la cirugía endodóntica asociada a la apicectomía como una técnica altamente eficiente para los casos de fracaso del tratamiento endodóntico convencional. A través de los estudios seleccionados, se encontró que cuando es indicada adecuadamente y ejecutada correctamente por los profesionales, esta técnica permite un tratamiento eficiente. Además, es fundamental que los profesionales que realizan esta técnica cuenten con los conocimientos y habilidades necesarias, además del uso de materiales adecuados que puedan garantizar los mejores resultados terapéuticos.A cirurgia parendodôntica é um procedimento utilizado nos casos crônicos de infecção periapical persistente, além de casos de canais radiculares calcificados onde não se obteve sucesso no tratamento endodôntico convencional. Essa técnica pode estar associada a apicectomia para os casos em que é necessário apicetomizar uma raiz, sendo considerado um procedimento muito invasivo. Ainda assim, é uma modalidade que recebe grande destaque entre os procedimentos endodônticos, uma vez que visa remover as bactérias presentes na região apical, além de impedir que outros microrganismos possam adentrar novamente nos tecidos apicais do canal. Diante da importância dessa técnica terapêutica, o presente estudo teve como objetivo discutir as impressões contidas na literatura em relação à utilização da apicectomia na cirurgia parendodôntica, discorrendo sobre suas indicações e importância na prática clínica atual. O estudo foi desenvolvido através de uma revisão narrativa da literatura, com a seleção de estudos publicados no período de 2020 a 2024 e disponibilizados nas bases de dados SCIELO, Publimed e Google Acadêmico. A partir da aplicação dos critérios de pesquisa foram selecionados oito estudos, os quais reforçam a importância da cirurgia parendodôntica associada à apicectomia como técnica altamente eficiente para os casos de insucesso do tratamento endodôntico convencional. Através dos estudos selecionados verificou-se que quando indicador de forma adequada e executado com correção pelos profissionais, essa técnica permite um tratamento eficiente.
