Summary: The editorial focuses cancer burden in Brazil and worldwide, contextualizing the major issues involved in the estimates of incidence of the several types and scientific knowledge acquired so far in terms of prevention. It highlights the necessity to invest in public health actions that should be considered by the political and health authorities. Introduces arguments that strengthen the construction of a cancer prevention and control policy grounded on the existence of several etiologic pathways resulting from the accumulation and integration of exposures to different lifelong risk factors. Based on the well-succeeded experience with tobacco control in Brazil, it indicates the importance of controlling prevalent modifiable risks exposure with effective prevention strategies supported by epidemiologic studies that address socioeconomic inequalities and allow future comparisons and improvement of health surveillance.

Realza la necesidad de invertir en acciones de salud pública que deben ser consideradas por las autoridades políticas y sanitarias. Presenta argumentos que refuerzan que la construcción de una política de prevención y control del cáncer debe estar basada en la existencia de innumerables vías etiológicas resultantes de la acumulación y de la integración de exposiciones a diferentes factores de riesgo a lo largo de la vida. A partir de la muy exitosa experiencia con el control del tabaquismo en el Brasil, señala la importancia del control de exposiciones de riesgo prevalentes y que pueden ser modificadas con estrategias efectivas de prevención amparadas en estudios epidemiológicos que consideren las desigualdades socioeconómicas y que permitan comparaciones futuras y mejoría de la vigilancia en salud.O editorial enfoca o peso do câncer no mundo e no Brasil, contextualizando as principais questões envolvidas na projeção da incidência dos diversos tipos, bem como no conhecimento científico que vem sendo acumulado em termos de prevenção. Realça a necessidade em investir em ações de saúde pública que devem ser consideradas pelas autoridades políticas e sanitárias. Apresenta argumentação que reforça que a construção de uma política de prevenção e controle do câncer deve estar embasada na existência das inúmeras vias etiológicas decorrentes do acúmulo e da integração de exposições a direrentes fatores de risco ao longo da vida.
