Envenomation by Bothrops snake in the City of Afuá, Ilha de Marajó, Pará State, Brazil

Summary: INTRODUCTION: Bothrops snakes are responsible for most of the envenomations in Pará State and Brazil, with acute inflammatory activity and coagulation changes as the main manifestations. OBJECTIVE: To report a severe case of envenomation by Bothrops snake occurred in the City of Afuá, Ilha de Marajó, Pará State, in 2015. CASE REPORT: Male of 15 years of age was bitten on the left leg while extracting palm heart from Euterpe oleracea (açaizeiro). The patient was admitted to the hospital 7 h after the snakebite referring to intense local pain, showing ascending edema to the thigh, ecchymosis, local bleeding, nausea, vomiting, epigastralgia, and altered urine color. RESULTS: Envenomation was classified as a serious accident and indicated 12 ampoules of antibotrope serum. Laboratory tests revealed leukocytosis, thrombocytopenia, coagulation disorders, and presence of red blood cells, proteins, leukocytes, and hemoglobin in the urine. At less than 48 h, the coagulation was normalized. In the evolution, bubbles and local necrosis appeared with the presence of purulent secretion. Metronidazole and chloramphenicol were used with good clinical response. The patient was discharged without severe sequelae after nine days. CONCLUSION: The description is intended to inform the scientific community and health professionals about the clinical manifestations and laboratory alterations caused by Bothrops envenomation, snakes found on Ilha de Marajó.

INTRODUÇÃO: As serpentes do gênero Bothrops são responsáveis pela maioria dos envenenamentos no estado do Pará e no Brasil, sendo a atividade inflamatória aguda e as alterações de coagulação suas principais manifestações. OBJETIVO: Relatar caso grave de envenenamento botrópico, ocorrido no município de Afuá, Ilha de Marajó, Pará, em 2015. RELATO DO CASO: Menor de 15 anos de idade, sexo masculino, picado na perna esquerda, enquanto realizava extração de palmito da palmeira Euterpe oleracea (açaizeiro). O paciente deu entrada no hospital 7 h após o envenenamento, referindo dor intensa local, apresentando edema ascendente até a coxa, equimose, sangramento local, náuseas, vômitos, epigastralgia e alteração na cor da urina. RESULTADOS: O envenenamento foi classificado como acidente grave e indicado 12 ampolas de soro antibotrópico. Os exames laboratoriais evidenciaram leucocitose, plaquetopenia, alteração de coagulação e presença de hemácias, proteínas, leucócitos e hemoglobina na urina. Com menos de 48 h, normalizou-se a coagulação. Na evolução, surgiram bolhas e necrose local com a presença de secreção purulenta. Foram utilizados metronidazol e cloranfenicol com boa resposta clínica. O paciente recebeu alta hospitalar sem sequelas graves, após nove dias. CONCLUSÃO: A descrição serve para dar conhecimento à comunidade científica e aos profissionais de saúde sobre as manifestações clínicas e alterações laboratoriais ocasionadas pelo envenenamento com serpentes do complexo Bothrops, encontradas na Ilha de Marajó.
