Comparison of Laparoscopic and Open Surgery in Cholecystectomy for Complicated Cholelithiasis

Resumo: Cholecystectomy, whether laparoscopic or open, is the standard treatment for complicated cholelithiasis. Laparoscopic surgery has gained popularity due to its benefits, such as shorter recovery time and reduced postoperative pain. This article aims to conduct a systematic review of the current medical literature, following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, to identify the efficacy and safety of laparoscopic surgery compared to open surgery in cholecystectomy for complicated cholelithiasis. The research was conducted in the PubMed, Cochrane, LILACS, and SciELO databases, covering the last five years (2018-2023). The search strategy included the terms “Laparoscopic Cholecystectomy,” “Cholecystectomy,” and “Cholelithiasis,” with five studies selected to compose the systematic review. The results obtained demonstrated that laparoscopic surgery is associated with a lower rate of postoperative complications, shorter hospitalization time, and faster recovery compared to open surgery. However, open surgery is still preferred in cases of complex anatomy or severe complications. It was concluded that laparoscopic surgery is a safe and effective option for most cases of complicated cholelithiasis, providing significant benefits in terms of recovery and patient quality of life.

La colecistectomía, laparoscópica o abierta, es el tratamiento estándar de la colelitiasis complicada. La cirugía laparoscópica ha ganado popularidad debido a sus ventajas, como el menor tiempo de recuperación y la reducción del dolor postoperatorio. Este artículo pretende realizar una revisión sistemática de la literatura médica actual, siguiendo las directrices PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), para identificar la eficacia y seguridad de la cirugía laparoscópica comparada con la cirugía abierta en la colecistectomía para la colelitiasis complicada. La búsqueda se realizó en las bases de datos PubMed, Cochrane, LILACS y SciELO, abarcando los últimos cinco años (2018-2023). La estrategia de búsqueda incluyó los términos «Laparoscopic Cholecystectomy», «Cholecystectomy» y «Cholelithiasis», y se seleccionaron 5 estudios para conformar la revisión sistemática. Los resultados mostraron que la cirugía laparoscópica se asocia a una menor tasa de complicaciones postoperatorias, una hospitalización más corta y una recuperación más rápida en comparación con la cirugía abierta. Sin embargo, se sigue favoreciendo la cirugía abierta en casos de anatomía compleja o complicaciones graves. Se llegó a la conclusión de que la cirugía laparoscópica es una opción segura y eficaz para la mayoría de los casos de colelitiasis complicada, ya que proporciona beneficios significativos en términos de recuperación y calidad de vida de los pacientes.A colecistectomia, seja por via laparoscópica ou aberta, é o tratamento padrão para colelitíase complicada. A cirurgia laparoscópica tem ganhado popularidade devido aos seus benefícios, como menor tempo de recuperação e redução da dor pós-operatória. Este artigo tem por objetivo realizar uma revisão sistemática da literatura médica vigente, seguindo as diretrizes PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), para identificar a eficácia e segurança da cirurgia laparoscópica em comparação com a cirurgia aberta na colecistectomia para colelitíase complicada. A pesquisa foi conduzida nas bases de dados PubMed, Cochrane, LILACS e SciELO, abrangendo os últimos cinco anos (2018-2023). A estratégia de busca incluiu termos “Laparoscopic Cholecystectomy”, “Cholecystectomy” e “Cholelithiasis”, sendo selecionado 5 estudos para compor a revisão sistemática. Os resultados obtidos demonstraram que a cirurgia laparoscópica é associada a uma menor taxa de complicações pós-operatórias, menor tempo de hospitalização e recuperação mais rápida em comparação com a cirurgia aberta. No entanto, a cirurgia aberta ainda é preferida em casos de anatomia complexa ou complicações graves.
