THE EFFICACY OF TRANSCRANIAL MAGNETIC STIMULATION IN TREATING TREATMENT-RESISTANT DEPRESSION: An integrative literature review

Summary: Major Depressive Disorder (MDD) is alarming due to its high mortality rates and its prevalence in the world population, and, once it does not respond adequately to traditional treatment, the disorder can become a Treatment Resistant Depression (TRD). Among the recommended neurostimulation methods for treatment, Transcranial Magnetic Stimulation (TMS) stands out, as a non-invasive neuromodulation technique directed to the left dorsolateral prefrontal cortex (DLPFC). The main objectives of the work were to evaluate the efficacy of TMS in the treatment of TRD, to investigate its association with other treatments and to investigate the scope of existing studies on the subject. The present Integrative Literature Review extracted data from the SciELO, PubMed and Scopus databases, with the descriptors "Transcranial Magnetic Stimulation", "Depression" and "Efficacy", including experimental, clinical, double-blind, single-blind, placebo-controlled, cohort studies, among others. It was observed among the selected articles that, in the studied populations (between 12 and 78 years), the use of TMS for the treatment of TRD is effective and safe for reducing symptoms, with different applications being tested (application frequency, intensity and time) and their interactions with antidepressants, presenting no contraindications. The results found corroborate with other more recent studies on the subject. Finally, TMS is an effective technique for the treatment of TRD in a safe way for different populations, being recommended for the reduction of depressive symptoms and improvement of the quality of life of those living with the disorder.

Además, cuando no responde adecuadamente al tratamiento tradicional, puede convertirse en una Depresión Resistente al Tratamiento (TRD). Entre los métodos de neuroestimulación recomendados para su tratamiento, se encuentra la Estimulación Magnética Transcraneal (EMT), una técnica de neuromodulación no invasiva dirigida al córtex prefrontal dorsolateral izquierdo (DLPFC). Los principales objetivos de este trabajo fueron evaluar la eficacia de la EMT en el tratamiento de la TRD, investigar su asociación con otros tratamientos y explorar la amplitud de los estudios existentes sobre el tema. Esta Revisión Integrativa de la Literatura extrajo datos de las bases de datos SciELO, PubMed y Scopus, utilizando los descriptores "Transcranial Magnetic Stimulation", "Depression" y "Efficacy", e incluyendo estudios experimentales, clínicos, doble ciego, simple ciego, controlados con placebo, de cohorte y otros. Se observó en los artículos seleccionados que, en las poblaciones estudiadas (entre 12 y 78 años), el uso de la EMT para el tratamiento de la TRD es eficaz y seguro para reducir los síntomas, habiéndose probado diferentes aplicaciones (frecuencia de aplicación, intensidad y tiempo) y sus interacciones con antidepresivos, sin presentar contraindicaciones. Los resultados encontrados corroboran con otros estudios más recientes sobre el tema. Finalmente, la EMT es una técnica eficaz para el tratamiento de la TRD de forma segura en diferentes poblaciones, siendo recomendada para reducir los síntomas depresivos y mejorar la calidad de vida de quienes viven con el trastorno.O Transtorno Depressivo Maior (TDM) se mostra alarmante devido às altas taxas de mortalidade e a sua incidência na população mundial e, uma vez que não surte resposta adequada ao tratamento tradicional, o transtorno pode vir a se tornar uma Depressão Resistente a Tratamento (Treatment Resistant Depression - TRD). Os principais objetivos do trabalho consistiram em Avaliar a eficácia da TMS no tratamento da TRD, averiguar a sua associação a outros tratamentos e investigar a amplitude de estudos já existentes referentes ao tema. A presente revisão integrativa da literatura extraiu dados das bases SciELO, PubMed e Scopus, com os descritores “Transcranial Magnetic Stimulation”, “Depression” e “Efficacy”. Foi observado dentre os artigos selecionados que, nas populações estudadas (entre 12 e 78 anos), o uso da TMS para o tratamento da TRD se dá de forma eficaz e segura para a diminuição dos sintomas, sendo testadas diferentes aplicações (frequência de aplicação, intensidade e tempo) e suas interações com antidepressivos, não apresentando contra indicações. Os resultados encontrados corroboram com outros estudos mais recentes a respeito da temática.
