Summary: | Paralabral cysts, also named paraglenoid labral cysts, are just-articular cystic lesions that usually show a close relationship with the glenoid labrum – they may represent synovial cysts, ganglions cysts, or pseudocysts. Paralabral cyst of the shoulder joint can be observed in 2-4% of the general population, particularly in men during the third and fourth decade. In 79-89% they are related to labral tears, especially SLAP lesions. The symptoms of a paralabral cyst may be the result of rotator cuff disease, labral disease, and entrapment syndrome from compression of the suprascapular nerve (rare cause – cyst must be larger than 1 cm), instability, or a combination of these conditions. Diagnosis is usually made using MRI, with or without arthrography, although ultrasound and computed tomography (CT scan) also have been used for diagnosis. O cisto paralabral do ombro pode ser observado em 2–4% da população geral. Em 79–89% dos casos, apresenta relação com a rotura labral, especialmente o tipo lesão do lábio superior de anterior para posterior. Objetivo: Relatar um caso e revisar a literatura acerca desta lesão incomum. Relato do Caso: Homem de 43 anos que refere dor contínua no ombro esquerdo, principalmente após o trabalho. Não apresenta limitação de movimentos ao exame físico, apesar de relatar sentir dor. A ressonância magnética demonstra dois cistos paralabrais. Conclusão: Relatamos um caso de cisto paralabral, lesão inicialmente insuspeita à anamnese e ao exame físico, diagnosticada por exames de imagem. A ressonância magnética comumente diagnostica o cisto paralabral em pacientes com dor crônica.Palavras Chave: Imagem por ressonância magnética, Articulação do ombro, Lesões do ombro ABSTRACT: Introduction: Paralabral cysts, also named paraglenoid labral cysts, are just articular cystic lesions that usually show a close relationship with the glenoid labrum — they may represent synovial cysts, ganglions cysts, or pseudocysts. Paralabral cysts of the shoulder joint can be observed in 2–4% of the general population, particularly in men during the third and fourth decades. In 79–89% they are related to labral tears, especially superior labrum from anterior to posterior (SLAP) lesions. Objective: To report a case and review the literature about this uncommon lesion. Case report: A 43-year-old man reporting continuous pain on the left shoulder, mainly after work. He did not show movement limitations on physical examination, despite claiming to feel pain. A magnetic resonance demonstrated two paralabral cysts. Conclusion: We report a case of paralabral cyst, an injury initially unsuspected by anamnesis and physical examination, diagnosed by imaging tests.
