Antimicrobial Resistance: formulation of the response in the global health context

Summary: Antimicrobial Resistance (AMR) has proved to be a major public health problem at the global level. This paper examined the formulation of the response to AMR negotiated through the World Health Organization (WHO) by its Member States. Related WHO reports and resolutions from 1998 to 2019 were analysed. The findings indicate that, from 2014 on, more robust conditions were established for approval of a Global Action Plan on AMR, encompassing the concept of One Health and involving otherinternational entities (FAO, OIE, WTO and Unep). Content analysis and various analytical frameworks, considering two economic sectors (the livestock and pharmaceutical industries), proved relevant to illustrating the complexity of the issue, reinforcing its global importance and acknowledging the extent of antibiotic use in animals and the gaps in technological innovation. As the WHO is not only an important agent for mobilizing the response to AMR at the global level, but – despite a context of de-funding – has guaranteed a budget for action in this area, it is concluded that the public health perspective should prevail in the response to AMR.

El propósito de este documento es analizar la formulación de la respuesta negociada por la OMS a la AMR por parte de sus Estados miembros. Se analizaron los informes y las resoluciones producidos en la Asamblea Mundial de la Salud. Los resultados indican que a partir de 2014 se establecieron las condiciones para la aprobación más sólida del Plan de Acción Mundial sobre AMR, que abarca el concepto de Salud Única e involucra a otros organizaciones internacionales (FAO, OIE, OMC y Codex Alimentarius). El análisis de los contenidos de dos sectores económicos sensibles, la agricultura y la industria farmacéutica, demostró ser relevante para ilustrar la complejidad del tema, reforzando su relevancia global, reconociendo el alcance del uso de antibióticos en animales y las brechas en la innovación de las nuevas tecnologías. También se considera que en el proceso de formulación de la respuesta a la AMR, a partir de 2014, se buscaron nuevos mecanismos para garantizar la adhesión de los Estados Miembros de la OMS al Plan de Acción Mundial acordado internacionalmente. A resistência a antimicrobianos (AMR) tem se revelado como um dos maiores problemas para saúde pública no nível global. O objetivo deste artigo foi analisar a formulação da resposta à AMR negociada no âmbito da Organização Mundial da SAíde (OMS) por seus Estados-Membros. Foram analisados os relatórios e resoluções produzidos na Assembleia Mundial da Saúde. Os achados indicam que a partir de 2014 foram estabelecidos condições de possibilidade para a aprovação do Plano de Ação Global em AMR de forma mais robusta, abrangendo o conceito de Saúde Única e envolvendo outros organismos internacionais (FAO, OIE, OMC e Codex Alimentarius). A análise dos conteúdos a partir de dois setores econômicos sensíveis – agropecuária e indústria farmacêutica – se mostrou relevante para ilustrar a complexidade tanto da temática, reforçando sua relevância global, reconhecendo a dimensão do uso de antibióticos em animais e as lacunas em inovação de novas tecnologias.
