Resumo: The segmental surgery has been an important treatment for the correction of malocclusions of the dental arches, which are not able to be treated by conventional orthodontics. The combination of orthodontics and orthognathic surgery is used to correct facial deformities from trauma or genetic origin. The malpositioned implant usually does not allow a functional and aesthetic prosthesis restoration; its removal, as an alternative to treatment, can cause major destruction of the alveolar ridge and, the segmental surgery is a type of treatment that improves the placement of the implant, preserves the alveolar bone and improves the gingival contours, allowing a more natural prosthesis. This article reports a clinical case step by step, in a orthognathic surgery was carried out as a treatment, in order to correct the bad positioning of an implant located on the 21 tooth, following seven years of monitoring. The result allows us to report that the segmental surgery is a very viable alternative for treatment.
