Facing the chronic non-communicable diseases, one of the a major concern has been the high rate of physical inactivity on the populations, once this issue has been considered an independent risk factor with the highest prevalence in the population . The World Health Organization (WHO) estimates that in each $ 1 invested in physical activity, other $ 3.20 is saved in medical costs. In recent years, the Brazilian Ministry of Health has promoted discussions and actions in order to set up the practice of physical activity in the Unified Health System (SUS), mainly at the Primary Health Care. Within this context, this study aims, through a case study, to describe the insertion process and the conceptual model of a physical exercise program in primary health care having the experience in Rio Claro County - SP, started in 2001. As a study methodology, it was decided to conduct a bibliographic search on the integration process of physical activities in the Brazilian Unified Health System and, through a case study, having as object the program of Exercise in Health Care Facilities, proposed by the Center for Physical Activity, Sport and Health ( NAFES ), the Department of Physical Education, UNESP - Rio Claro, and the Health Department of the Municipality of Rio Claro - SP, rescuing the process of creating, implementing and developing the program in Rio Claro County Primary Health Care, using semi structured interviews. The interviews were analyzed using the technique of content analysis and methodology Analysis of Collective Subject Discourse. Analyses of oral discourses present reports as social representations of the focus group, identified the historical, political and social context of the time of preparation and implementation of the program.
