Resumo: | During pregnancy, there changes that favor the emergence of systemic changes with repercussions on oral health, with a tendency to vomiting and nausea, in addition to greater vascularization of the periodontium and hypersecretion of the salivary glands. In addition to periodontal and saliva modification, there are also alterations in the oral microbiota and cellular metabolism, allowing the appearance of the gravid tumor. Considering that such oral changes can trigger complications such as low birth weight, premature birth and even infant mortality. The present study aimed to understand the access to prenatal dental care for Brazilian and Brazilian pregnant women residing in the municipality of Foz do Iguaçu. This is an exploratory descriptive study, which used quantitative and qualitative approaches. Data collection was carried out in the first half of 2022. The quantitative research had as its primary source dental records and information acquired through the Health Information System for Primary Care of the Ministry of Health, referring to the indicator of proportion of pregnant women with dental care performed between the years 2011 to 2021, in all health units with dentistry services and the Mother and Child Center and analyzed with descriptive statistics. Regarding the qualitative research, pregnant women in prenatal care linked to the units were interviewed, as well as dentists, doctors and nurses who work in the health units selected and distributed in the five Sanitary Districts of the municipality of Foz do Iguaçu-PR. The data were based on the assumptions of the content analysis proposed by Bardin. The research identified that 68.56% (n=301) of the pregnant women assisted at the Mother and Child Center are paraguayans and/or brasiguaias, with the majority 56.95% (n=250) aged between 20 and 30 years old, and the majority Dental prenatal care in this location began with the implementation of the dental office in 2021, assisting 97.14% (n=102) of pregnant women in that year. In the Basic Health Units, the age group found has the same proportion as in the Mother and Child Center, with 53.13% (n=27,867) aged 20 to 30 years. Regarding dental prenatal care, the absence of this assistance was evident. The lowest indicator found was 6% (n=142) in the 1st quadrimester of 2017. However, the other years also presented similar indicators. Even with the insufficiency of the data found, only in 2021, 50% (n=483) of the pregnant women received dental prenatal care, precisely the year following the implementation of the Prevent Brazil Program, whose objective is to increase access, through the new financing model. The qualitative results identified six categories: Facilitated access to regular and dental prenatal care; Lack of knowledge about the importance of dental prenatal care; Disjointed access to health services in the perception of professionals; Absence of professional collectivity in the assistance to pregnant women; Perceptions of the multidisciplinary team on the importance of dental prenatal care and changes in the oral cavity during the gestational period; COVID-19 pandemic and lack of (attention) in pregnant women care. The study showed the low adherence to dental prenatal care compared to the usual prenatal care and that there are still barriers to accessing and using the service, which are related to the lack of knowledge of the pregnant woman and of part of the health professionals. |
