Análise dos aspectos funcionais dos espermatozoides e dos níveis de peroxidação lipídica no plasma seminal de pacientes portadores de tumor de células germinativas de testículo pré e pós-orquiectomia

Resumo: | Objective: To verify if quality of spermatozoa from men with testicular germ cell tumors is better before or after orchiectomy. Methods: This prospective study was carried out including 24 patients with testicular germ cell tumors. The patients provided one semen sample before they were submitted to unilateral orchiectomy, and one other semen sample 30 days after the surgery. After collection by masturbation and liquefaction, an aliquot of the semen sample was used for semen analysis and another was used to evaluate sperm mitochondrial activity, DNA fragmentation and acrosomal integrity. The remaining volume was centrifuged and the supernatant seminal plasma was used to evaluate lipid peroxidation levels. Mitochondrial activity was determined by a colorimetric stain (3,3? ? diaminobenzidine ? DAB) and classified as DAB I (all mitochondrial active), DAB II (more than 50% mitochondrial active), DAB III (less than 50% mitochondrial active) and DAB IV (all mitochondrial inactive). Sperm DNA fragmentation was assessed by an Alkaline Comet assay, in which sperm were classified as high DNA integrity (class I and II) to high DNA fragmentation (class III and IV). Acrossome integrity was determined by a fluorescent probe (PNA ? FITC) and assessed by fluorescent microscopy. Lipid peroxidation levels were quantified by measuring thiobarbituric acid reactive substances (TBARS) levels, and were described in TBARS nmol/mL. Pre and Post-orchiectomy sample were compared using a paired Student?s T test (normal distribution) or a paired Wilcoxon test, when appropriate (p?0.05). Results: No significant difference was observed in semen analysis. A significant decrease in DNA fragmentation (p=0.019) and lipid peroxidation (p=0.014), and an increase in mitochondrial activity (p=0.048) was observed after orchiectomy. Conclusions: Based on our findings, the semen quality from men with testicular germ cell tumors is better quality after orchiectomy. |
