Comparação entre terapia comportamental e acupuntura na incontinência urinária feminina por hiperatividade do detrusor

Resumo: OBJECTIVE: To compare the results of behavioral therapy and acupuncture in women with detrusor overactivity, through four assessment methods: the voiding diary, questionnaire about quality of life in urinary incontinence "King's Health Questionnaire" questionnaire of quality of life for overactive bladder ("OAB" instrument - V8 ?), and the Absorbent test "PAD TEST". METHODS: This study is a randomized, controlted trial. The patients agreed to treatment proposal and signed the free and informed consent. 120 women were selected from March 2011 to November 2014 for 12 weeks of treatment, once a week. The assessment instruments were applied before and after treatment and evaluated with Voiding Diary of seven days, one hour "Pad Test", quality of life Questionnaire in Urinary Incontinence - "KHQ" and the quality of life Questionnaire for overactive bladder "OBA" - V8 ?. The volunteers were divided into 2 groups (Behavioral therapy and acupuncture). Location: It was carried out at the Federal University of São Paulo - Paulista School of Medicine (UNIFESP - EPM) RESULTS: For voiding diary, behavioral therapy patient was more effective in reducing symptoms of urgency (down 82.81%), urge incontinence (down 79.65%), nocturia (down 73.89%), effort losses ( reduction of 65.83%) and reduction in absorbent exchanges (down 92.86%) (P-value <0.0001) when compared to acupuncture. In the quality of life Questionnaire "King's", the behavioral therapy group had lower average scores after treatment for "physical limitations" and "emotions" (P-value <0.0001). In relation to the items "general health", "incontinence impact," "limitations in daily activities," "physical limitations", "emotions", "sleep and disposition" and "severity measures", Behavioral therapy was also more effective . Concerning, the V8 questionnaire, ali patients who were examined before treatment had a high rate for overactive bladder. In acupuncture group, only 1 patient (5:56%) improved in score in Behavioral group, 9 patients (40.91%) improved after treatment. CONCLUSION: Taking into consideration the four instruments of assessment, the patients of the behavioral therapy group showed better results when compared to the acupuncture group.
