Resumo: Purposes: To evaluate the cost-effectiveness of the population screening programs and treatment of retinopathy in diabetic patients. Methods: It was evaluated the retinopathy stage and its costs for the diagnosis and treatment among diabetic patients who were cared for at the project for visual screening (group I) and those who were cared for at the service of ophthalmology “Altino Ventura Foundation – Recife, Pernambuco – Brasil” (group II). Results: The proliferative retinopathy form was more prevalent among patients of group II (group I 22.2% vs group II 49.1% - p < 0.0001). Diabetic macular oedema was more prevalent among patients of group II (group I 13.8% vs group II 26.7% p = 0.0399). The indication for retinopathy treatment was more encountered among patients of group II (group I 26.2% vs group II 58.7% - p < 0.0001). In group II, the median value of the real cost was higher among patients with the proliferative form of the disease (non-proliferative form R$ 115.79 vs proliferative form R$ 171.79 p < 0.0001). Comments: Due to the fact of finding a smaller number of patients with the serious form of retinopathy in group I, and the estimate of the reduction in the cost of the treatment of these patients, it is suggested that the projects for visual screening of diabetic must be stimulated with the aim of not only reducing the cost of the treatment of these patients, as well as to provide a better medical and social results for diminishing the blindness possibility.
