Resumo: Introduction: Transverse rectus abdominis musculocutaneous (TRAM) flap is considered an important surgical option for breast reconstruction but may present necrosis, especially in patients with risk factors. Perspectives on cell therapy with adipose derived mesenchymal stem cells (ADSC) for neovascularization and reducing necrosis of TRAM flap, originated this study. Objective: The objective of this study was to determine the effect of adipose derived mesenchymal stem cell in TRAM flap?s rat model viability. Methods: Twenty-four Wistar rats were distributed into three groups (n=8). The caudal pedicled TRAM flap was harvested in all animals and it was the only procedure in the group I (TRAM). In groups II (Alpha MEM) and III (Stem Cell), with the TRAM flap procedure was performed intradermal injection Alpha MEM medium culture and the Alpha MEM medium culture with two million mesenchymal stem cells with the fluorescent marker Dil, respectively. We evaluated the percentage of necrosis, neovascularization by immunohistochemistry with HHF-35 antibody and the distribution of stem cells in TRAM flap by fluorescence microscopy. Results: TRAM group, Alpha MEM group, and Stem Cell group showed a mean percentage of necrosis of 50,42%, 53,57%, and 23,36%, respectively (p=0,005). TRAM flap?s immunohistochemical analysis showed a significant increase in number of vessels in the zone IV to group III. Conclusion: Adipose derived mesenchymal stem cells improved TRAM flap viability and the number of the vessels in the zone IV of flap, in rats.
