Resumo: This study aimed to analyze the formal organization of work processes in the four Federal University Hospitals of RS, Brazil, from the perspective of quality and safety of patients and professionals. This is a descriptive field research of qualitative and quantitative nature. Data were collected through a semi-structured survey instrument, completed by four professionals who work in quality services or similar of those institutions in the months of June and July 2014. The ethical principles for the conduct of research with human subjects set forth in Resolution 466/12 / CNS were respected. The integrated analysis of the results allowed to argue that the University Hospitals, are progressively sensitive to the importance of modeling of work processes to achieve a standard of care. All considered important the formal organization of work processes (manual labor) and understood that this positively influences, the safety of patients and professionals. The data disclosed the existence of formal structure of standardization of work processes, but in only one HU there is a structured quality service and certification in quality program. There are deficiencies in the use of quality tools, and management, widespread in the literature. As a major difficulty in the organization of work processes appeared to resistance from employees, followed by excess of other activities and lack of infrastructure. It is understood that to improve the institutional security the alternative may be the adoption of a quality program, the process being coordinated by a quality team, implemented in a participatory manner in order to reduce the resistance to new and changes. It is essential that quality goals are part of the institutional policy, to be seen as common goals to all, with participation of managers. It is noticed that the HUs have many challenges, mainly related to personnel management. Must be developed managers able to "buy that idea", that is, committed to quality management.
