Resumo: We seek to understand the speeches that have produced the clinic with severe cases in a Psychosocial Day-Care Centers in the central region of Rio Grande do Sul State. In this research were investigated the discourses that make up and determine serious cases, debating how the conceptions of health and mental health impact in the clinic with these cases, analyzing the conceptions of clinic-watch produced from the logic asylum and investigating the institutional speeches about the clinic of psychosis. To achieve the goals we conducted a case study on a CAPS I, building a Board Diary. The method to address the data was the Institutional Analysis of the speech, Marlene Guirado. The theoretical framework for understanding the theme was directed by the construction of two articles, the first: Speeches about Serious Case in Psychosocial Day-Care Centers: The health-disease process; And the second: Speeches from the Clinic with Severe Cases in Psychosocial Day-Care Centers. The materials of the two articles synthesize the understanding of the research on how the cases are being considered serious and the clinical model offered in these cases.
