Resumo: Introduction: The process of population aging on the world stage is one of the phenomena of great impact of the 21st century, with significant repercussions for the lives of the elderly, which can be effected by the presence of fragilities, arising from the interaction between biological, psychological and social factors. Thus, immobility is characterized by biological and physiological changes that occur in all systems of the body, and one of its most serious consequences is the Immobility Syndrome, which affects individuals with disabling diseases, capable of causing damage to health or even the death of the elderly. Objectives: Build an instrument to assess immobility and immobility syndrome in the elderly; describe the scientific evidence about the factors that can cause immobility or immobility syndrome in the elderly; verify the content validity of the instrument through a committee of judges; perform the semantic evaluation of the instrument by a sample of health professionals; to identify the main physiological and functional changes related to immobility and immobility syndrome in a sample of elderly people. Method: Methodological, descriptive study, developed in six stages: integrative literature review about the factors that can cause immobility or immobility syndrome in the elderly; development of a proposal for an instrument to assess immobility and immobility syndrome in the elderly; instrument content validation by a committee of judges; semantic validation of the instrument by health professionals, resulting in the final version of the technological product; and, identification of the main physiological and functional changes related to immobility and immobility syndrome in the sample of elderly participants in the study.Results: In the integrative review, 10 studies were selected that addressed the factors that can cause immobility or immobility syndrome in the elderly. Most of them were produced in Brazil, between 2000 and 2019. In them, it was evidenced that the causes that compromise the mobility of the elderly are multifactorial, contributing to the elaboration of the initial proposal of the instrument and evaluation of immobility and immobility syndrome in the elderly . In the stages of content and semantic validation, there was agreement by at least 80% of the evaluators, who considered the instrument to be good, easy to apply, suitable for use by any health professional and important to assess immobility and immobility syndrome. in the elderly. The application of the Instrument for the Assessment of Immobility and Immobility Syndrome in the Elderly (IAISII) resulted in the identification of 46.1% of the elderly, considered to have the immobility syndrome. Discussion: In order to meet the considerations of the judges and health professionals, adjustments and additions were made to the respective items of the instrument: demographic data, general assessment and systems (musculoskeletal, cardiovascular, respiratory, gastrointestinal, genitourinary and integumentary). Conclusions: An instrument was built to assess immobility and immobility syndrome in the elderly. It is important to emphasize the importance of new studies on this topic, as well as the need for health professionals to understand the physiological changes caused by aging, and thus, to assess the presence of immobility and immobility syndrome, in order to guide care for the patient. elderly in an agile and efficient way, especially those with impaired mobility.
