Resumo: The model of functionality of the World Health Organization (WHO) allows an evaluation of the functional / incapacity in a globalized and standardized way. It can be used in various conditions of deficiencies that generate difficulty in carrying out daily activities. People with mild cognitive impairment (MCI) and dementia demonstrate functional deficits that compromise quality of life and autonomy. In MCI, there are inconsistencies regarding the diagnostic criteria and the characteristics of functional declines, with few functional assessment scales for this clinical group and those that are currently used have inefficient psychometric properties. There is no functional assessment tool that is a reference for MCI and dementia. The measures of functional performance evaluation, which are applied in real context, seem to be more sensitive in detecting functional declines in MCI. In view of this, the study aimed to build and validate a direct evaluation scale of the functionality for people with MCI and mild dementia. For this was used a sample of 43 participants, being 14 without cognitive impairment, 15 with MCI and 14 with dementia, the age ranged from 58 to 85 years and the subjects of clinical syndromes were diagnosed by a clinical group composed of Geriatrician, Neurologists and neuropsychologists. It was applied to DAD, in all the research subjects informants and then the objective evaluation scale of the functional performance (FOPAS) was used in the groups. The analysis of the data was performed using the IBM-statistical Package for the Social Sciences version 25 to perform reliability tests, exploratory factorial analysis, Pearson correlation, ANOVA of one factor and the square chi for categorical variables; The program Medcalc version 18.2.1 was used to trace the Curve Roc and define the sensitivity and specificity of FOPAS. The results showed that FOPAS is an instrument for evaluating complex activities of reliable and valid functionality to differentiate healthy subjects from people with MCI and dementia. The domains most committed in the crossings between MCI and dementia were management with finance, food and drug use, moreover, the CCL group committed more errors of action than controls but less that the participants with dementia. The findings of this study were promising, demonstrating the FOPAS as a sensitive tool in detecting the differences between the clinical groups studied, and can be used in clinical practice and research to assist diagnoses and guide interventions.
