Resumo: | INTRODUCTION: Family Health is a strategy of the care model reorientation which is performed due to the implementation of multiprofessional teams in basic health units. These teams are responsible for taking care of a definite number of families located in a delimited geographical area, acting in health promotion and prevention, combat to the most frequent diseases and grievances as well as health maintenance of this community. As part of this context, the nursing professional is involved in activities such as education, planning, organization and evaluation of health actions, nursing visit, among others. The nursing visit can be an excellent tool for controlling the degenerative chronic problems, such as Systemic Blood Hypertension (SBH) which is considered a great and serious public health problem. The nursing history constitutes a fundamental tool to make viable the implementation of the first phase of the nursing visit: the data collection. OBJECTIVE: To construct and validate an instrument for nursing visit to the hypertense patients cared at the Health Family Units, based on Horta´s Theory of Basic Human Needs. METHODOLOGY: It is a methodological research which was accomplished at the Family Health Units (FHU), in the town of Cabedelo PB. In regard to the population and sample, the assistant nurses and professors, who act in the FHUs, and by means of the Statement of Consent, accepted to take part in this study. The research was developed in five phases: identification of the empirical indicators of the basic human needs in hypertense individuals; organization of the nursing visit instrument for hypertense patients; development of statements concerning the diagnoses/results and nursing interventions from the clinical indicators, of the Basic Human Needs, which were identified and validated; validation of an instrument for Nursing Assistance Systematization, containing all the nursing process phases; instrument operationalization. RESULTS: A large literature review and an analysis of the hypertense patients´records, in order to identify their needs, were carried out. This was based on Horta´s proposal, resulting in the attainment of specific indicators, for hypertense individuals, which were validated by assistant nurses and professors. Afterwards, diagnoses/results statements as well as nursing interventions were developed and used in the instrument construction, and thus they were validated. After such validation, the third version of the instrument was made up and applied to hypertense patients, by the nurses at the FHUs, with the aim to verify the feasibility of its operationalization. As a result, the final version of the viable instrument to be used in hypertense individuals was obtained. FINAL COMMENTS: With the construction of the nursing visit instrument for hypertense individuals, advancement as regards the implementation of the Nursing Assistance Systematization, at the Health Basic Units, is expected to occur. Furthermore, it is assumed that this will promote a higher quality with respect to assistance and valorization of the nurse s role in the institution as well as more efficiency, autonomy and scientificity to the profession. The clients´data register will aid the nursing team and, consequently the communication will be more effective among these professionals who care for the hypertense client. |
