CONSTRUÇÃO DE UM PROTOCOLO TERAPÊUTICO DE LASERTERAPIA PARA TRATAMENTO DE LESÕES MAMILARES EM LACTANTES

Resumo: One of the causes for abandoning breastfeeding is nipple trauma. Many postpartum women give up breastfeeding due to discomfort and pain, and early cessation can cause problems for both the postpartum woman and the child. Thus, the use of an assistive technology, such as a laser therapy protocol for the treatment of nipple injuries, can provide nurses with safe and efficient care for breastfeeding women, in addition to contributing to ongoing health education actions. The use of this technology enables nurses to develop care based on scientific evidence. Objective: To carry out an integrative literature review, with a view to elucidating existing care protocols in promoting the treatment of nipple fissures in infants. Subsequently, and based on the findings, the objective was to construct a therapeutic protocol of laser therapy for the treatment of nipple fissures in infants. Method: The methodological route was conducted through a qualitative research. The protocol was created from an integrative literature review. The search for publications was carried out in the following databases: Cochrane Library Central, Latin American and Caribbean Literature in Health Sciences (LILACS), National Center for Biotechnology Information (PubMed), Scopus, Scientific Electronic Library Online (SciELO), Web of Science, Science Direct, Coordination for the Improvement of Higher Education Personnel (CAPES), Virtual Health Library (BVS). The established time frame was from 2010 to 2022. After surveying the studies through the integrative literature review, a protocol was constructed for the use of laser in promoting the treatment of nipple fissures in breastfeeding women. Results: It was identified that the main treatments used for nipple fissures are: use of phytotherapy, lanolin, ointments, phototherapy and laser therapy. Thus, the therapeutic protocol of laser therapy for the treatment of nipple injuries, constructed as a product of this study, sought to standardize actions regarding the use of low-intensity laser therapy, with a view to stimulating wound healing, pain relief and reduction of inflammatory processes in breast lesions and fissures. The care protocol is aimed at nursing mothers in a Nursing Office and designed for the effective use of the nursing process in care actions for the treatment of nipple injuries and aims at standardizing actions regarding the use of low-intensity laser therapy, stimulating wound healing, pain relief and reduction of inflammatory processes in nipple injuries. Final Considerations: The literature review showed the benefits of treating nipple injuries in infants with low-intensity laser therapy, as well as the importance of using protocols for treating fissures with phototherapy. Protocol-based solutions, implemented in nursing offices, require training of professionals in the area. The protocol constructed through this study is a relevant technical product, according to the Capes Technical Production Qualis, described in item 3, with T1 extract and score 100. The protocol is available in digital media and can be accessed free of charge.
