Resumo: Introduction: Necessary to raise awareness among managers and health professionals. Breastfeeding when carried out in the first hour of life, from birth, maintained exclusively until 6 months and complemented until the age of two years or more, shows great transformative potential in the growth, development and prevention of diseases in childhood and adulthood . The theme in question is referenced in the National Agenda for Health Research Priorities, published in 2018, related to the maternal and child area (Axis 14). In view of the above, this study sought to develop an assistance and management tool to help improve maternal and child health, changing old concepts and reducing harmful and / or unnecessary practices, as recommended by the World Health Organization. prenatal care and, especially, in maternity wards and obstetric centers, they must be trained in the clinical management of encouraging breastfeeding and supporting mothers to start it as early as possible, according to what the Stork net and Humanization Policy. Scientific Objectives: To build and validate a breastfeeding assistance protocol in the first hour of life of the newborn; To sensitize and train the coordinators and professionals of the maternity hospitals that provide assistance to the newborn in the first hour of life, for the implementation of the assistance protocol, in the region of Vale do Taquari/RS. Method: The steps of the methodological research, developed from August 2019 to November 2020, were followed for the implementation of the assistance protocol. It was built and sent for content validation, by specialists from the multidisciplinary team in the breastfeeding area, among them, nurses, doctors, psychologists, teachers, breastfeeding consultants and biomedical doctors. Appearance validation concerns the applicability of the protocol in the study scenario, carried out by raising the awareness of managers and training professionals working in the Maternities of the 16th Regional Health Coordination of RS. Results: In order to achieve the first proposed objective, an integrative bibliographic review research was carried out on breastfeeding in the first hour of life, which theoretically supported the elaboration of the protocol, generating manuscript 1 of this study. To validate the content of the protocol, two cycles of the Delphi Technique were performed, with the participation of 21 specialists in the field of breastfeeding. The protocol was evaluated using a Likert scale and approved with Content Validity Index = 1.00. Regarding the second objective, the appearance validation took place with meetings to raise awareness of the proposal and present the validated protocol to the coordinators and heads of the maternity hospitals involved, followed by the training of assistance professionals with a view to its implementation in the region, generating the second manuscript, entitled “Assistance protocol in the first hour of life: from construction to implantation”. This process culminated with the co-responsibility of the municipal management and caused an intense movement of transformation in the work process of the Nursing professionals and other members of the multidisciplinary team. Final Considerations: The 'Assistance protocol in the first hour of life, with an emphasis on breastfeeding', presents itself as the most relevant and punctuated technical product, according to Capes' Qualis Produção Técnica, described in item 3, with Extract T1: Manual or Protocols (100 points). It will be available on digital media in Ebook format, with regional coverage, and can be used in other obstetric and neonatal care services. This protocol allows to direct, unify and humanize care practices in the newborn's first hour of life. The applicability of the protocol has already started and depends on the continuity of training for the performers, so it is suggested that monitoring and evaluation of its use be carried out in the periodic meetings of the multiprofessional teams. The implementation of the assistance protocol validated by specialists is configured as a management tool and a strategy to strengthen the best care practices. It is worth mentioning that this is a powerful device for changes in the maternity settings, as it includes the subjects involved, being them, managers and a multidisciplinary team, benefiting the mother / baby and companion.
