Depressão no pós-parto : cuidado de enfermagem para casais identificados com fatores predeterminantes e vulnerabilidade ao adoecimento

Resumo: | The families live new and complex experiences resulting from the childbirth due to the increase in the responsibilities and changes from this moment. To the parents, feelings of conflict, uncertainty, doubts and anxieties can arise, which are some triggering factor for mental suffering. Around 10 to 15% of mothers and 10% of fathers are susceptible to develop postpartum depression. The present study had as general aim to build a model of nursing care in the primary care to the promotion of mental health in pregnant couples, which had risk to develop depression after the childbirth. The specific aims were: to identify vulnerable couples to the development of depression after the childbirth based on the application of the nursing history (including genogram, ecomap and the EPDF scale); to develop interventions to minimize the potentiality of risk factor to the depression of pregnant after the childbirth; to evaluate the occurrence of depression after the childbirth in postpartum women after the intervention of the nurse; to evaluate the effectiveness of the actions developed to the promotion of mental health. Methodology: it is a qualitative study that used the Convergent Care Research method. Individual open and semi-structured interviews with three families were done, applying the nursing story, the Edinburgh postnatal depression scale and the construction of genogram and ecomap of the family. Three conversation wheels with a group of pregnant and health professionals were part of the data resource about the themes: (1) maternity and paternity, (2) assistance to the partum and prevention of obstetric violence, (3) postpartum and breastfeeding. The transcription of the recordings of the audio of the meetings (interviews and conversation wheels) was part of the empiric data of the research. Results: the research resulted in the construction and application of a model of nursing care in the primary care to the promotion of mental health for pregnant couples with risk of developing depression after the childbirth, offering greater visibility to the needs of the couples during the pregnancy, partum and postpartum periods. This also collaborated so the families and the health professionals could understand that postpartum depression is a serious health problem and that this affects several ranges of the families’ lives. The study reaffirmed the evidence that the depression may happen with anybody, but that there are people that have factors that favor the development of depressive disorder. In this way, instruments that allowed the identification of such factors during the pregnancy were used, as well as specific measures to offer support to the families that were more vulnerable to the develop this illness were built. |
