Resumo: The dynamics of public health services has increased the discussion of health care networks. Health System in the Municipality of Belo Horizonte (SUS-BH) is a network of health care hierarchical. To care for the elderly this network is translated into "flowchart senior care." The primary care professionals build a network to take care of the elderly concerning their interactions, in a context of facilities and barriers, aiming to meet the emerging demands to establish care. The aim of this study was to develop a theoretical model of elderly care in the Network of Health Care from Primary Health SUS-BH. And the specific objectives: to analyze the network of health care established in the Primary Health Care for the elderly SUS-BH; identify interactions established from the flow of senior care in the SUS-BH; analyze how interprofessional interactions and influence the process of inter-networks of health care for the elderly and comprehensive care to the elderly. It is a qualitative descriptive and exploratory in nature based on the Grounded Theory. Data collection occurred between November and December 2011, using two sample groups, whose instruments were the focus group and individual interviews, totaling 20 professionals APS and Rehabilitation Center of Social Health District Northwest, interrupting with data saturation. Respected to the ethical. Data analysis was concurrent with the collection, following the process of open coding, axial, selective, emerging the following categories: operating a network of care to the elderly in the APS, discussing the context of elderly care, barriers and realizing the new care facilities reflecting on professional interactions in the context of health care network for the elderly; seeking ways to manage care for the elderly; building a network of care to the elderly in the APS. These categories came to the central category: Network Health Care developed to care for the elderly in Primary SUS-BH, whose theoretical model was validated by the third sample group. The methodological rigor required of PDT and dynamic circularity make the model developed subject to criticism and suggestions for continuing improvement. Thus, one can not say that the model is built. There are opportunities for growth, to motivate the emergence of other concerns on the part of researchers and professionals who can (re) build (re) define professional practices and influence public policies to strengthen attention to elder care and qualify.
