Resumo: Introduction: The continuity of care for mentaly ill patients in community services has been seen, in the last years, not only as an aim to be gained, but also as a way to evaluate the quality of these services. In spit of the importance of continuity of care for the severely mentaly ill, this study aimed describe patienst enrolled in a Mental Health Reference Center (CERSAM) and analyze associated factors with continuity of care in health centers of Belo HorizonteMG. Method: A no-competitive prospective cohort study was carried out in two phases. The first one occurred in CERSAM located in Barreiro area in Belo Horizonte, MG,where socio-demographic and clinical variables were collected and the cross referrence for health centers of 214 patients was verified. The second phase, occurred in eight health centers located in CERSAMs Barreiro catchment area, where data were collected regardingcontinuity of care in this units. Descriptive and comparative statistical analysis were performed using Qui-square and Teste T. Results: Indicated, for the first phase, that CERSAM Barreiro assisted more young women, with psychosis diagnosis, that arrived to the service spontaneously, without previous psychiatric history. The patients stayed in CERSAM for 7,7 months and half was referred to health centers. Patients who stayed in the service more than 1 year had more chances to be referred for health centers. For the second phase, the results indicated that 1/3 of the referred patients were not found in the health centers visited. The patiens who were found, 62% continued in treatment. To return to CERSAM for a new consultation after the first referrence, and be referred 2 or more times were associated to the continuity of care. Any individual caracteristics were related to continuity of care. Conclusions: CERSAM Barreiro enrolled the clientele for which was destined, beingconfigured as a reference in the municipal scenery. A gap in continuity of care was verified when patients were cross-referred to health centers. In spit of it, the patients who were found in health centers after CERSAMs reference had have continuity of care. The dificults associated to continuity of care were more related to structural factors of health system than to patient´s characteristics.
