Avaliação da atenção primária à saúde no município de Belo Horizonte: estudo realizado entre profissionais de saúde aliado a estudo transversal de base populacional

Resumo: Primary health care (PHC) is understood as the gateway of the health system and place to provide continuous and integral care to the individual, in the context of family and community. In Brazil, PHC qualification has been occurring mainly through the Family Health Strategy (FHP) since 1994 and several investigations have shown that this strategy has been effective in improving health indicators. This study, conducted in the city of Belo Horizonte, aims to verify the distribution of APS attributes in the perception of managers and professionals, to determine the factors associated to PHC performance and to each one of these attributes and to which quality indicators from the point of view of the user they are associated. The research was based on the application of the Portuguese version of the Primary Health Care Tool (PCATool) questionnaire in all the Family Health teams (eSF) of Belo Horizonte and also in the database from the 2nd Adult Health Survey conducted in 2010. In the first study the independent variables were: resources available in the Primary Health Care Unit and characteristics of the health professionals and in the 2nd, the independent ones were absence of complaint regarding the health service, having received medical advice on diet, physical activity, alcohol or smoking, have had at least one medical consultation in the year with specialist, have performed at least three medical consultations in the year with generalist, performed one or more blood pressure measurements in the last two years, cholesterol determination in the last 5 years, achievement pap smear in the last 3 years and vaccination in the elderly. In the first study, the Prevalence Ratios (PR) and 95% confidence intervals were estimated by means of negative binomial regression and 2nd by Poisson regression. The APS of the city of Belo Horizonte presented good performance, mainly in relation to the attributes of the first contact, longitudinality and comprehensiveness. The main determinant of this performance is the availability of equipment and inputs, which was associated with the APS score and four of the seven attributes. The other factors associated with PHC performance were the presence of the physician at least 75% of the time of the week, a team with family health training and the number of teams at the UBS. The quality of PHC has an impact on the indicators studied and the attributes that showed the greatest association with these indicators were access and longitudinality.
