Resumo: | This dissertation presents an assessment of the contributing factors to job-related accidents in the Brazilian chemical industry. The methodology used is based on accident causation model named Tripod-Beta and aims, through a case study, by means of 11 basic risk factors (BRFs) the identification of the latent failures in a safety management system. From these latent failures, which are originated from strategic and management decisions, and, in its turn result into unsafe conditions in a workplace, it is possible to control, to reduce and to prevent the human errors occurrence. Management mechanisms that were implemented by the company which had taken part in this study are evaluated against key point indicators, as labour accident frequency rates, obtained from three industrial sites of the company located in different regions of Brazil. From the obtained results the author intends to analyze Tripod-Beta contribution to identify and to understand the real causes of job-related accidents in the chemical industry |
