Resumo: A strong sense of coherence (SCO) allows the indi vidual to face daily hardships as a result of the disease and stay healthy. People living with HIV / AIDS (P V HA) live with stress and experience stigma, which can compromise adherence to antiretroviral therapy (ART). In this context, we developed a study i n order to analyze the sense of coherence and adherence to antiretroviral therapy for people living with HIV / AIDS. This is a prospective longitudinal study developed in the infectious diseases clinic at the University Hospital Walter Cantídio. The sample consisted of 98 PLWHA in use of antiretroviral therapy. Data collection was performed in two stages, in 2012 and 2013, with an interval of 12 months. It used as instruments the characterization Form sociodemographic and clinic for people with HIV / AIDS, Antonovsky Sense of Coherence Questionnaire (QSCA) and the accession Assessment Questionnaire to antiretroviral treatment (CEAT - VIH ). Statistical analysis were applied the McNemar test, Wilcoxon, Mann - Whitney and Spearman correlation coefficient. The preva lence ratio, average, standard deviation, median, minimum and maximum values were calculated when needed. The ethical aspects were respected. Of the total respondents, the majority were male (62.2%), heterosexual (62.2%), act ive employment status (62.1%) , c atholics (67.3%) with mean age of 39, 5 (± 10.3) years. The ma le patients (p=0.034), homo /bisexual men (p =0.043), educated (0.022), with children (p=0.00 5), active employment status (p <0.001) an d lower incomes (p = 0.005). They showed greater adherence to ART. Regarding the SOC, male patients (p = 0.039), with active employment status (p = 0.010), inco me above two minimum wages (p= 0.033) and do not make use of psychiatric medication (p = 0.002) have highest average in QSCA. In analyzing the scores QSCA an d the total score of the CEAT - VIH , an assoc iation was seen in the first (p = 0.006) and in the second (p <0.001) evaluation. Through the sample analyzed in this study it was concluded that the PLWHA with better values for the sense of coherence have better score f or adherence to ART.
