Resumo: The changes that have occurred in the profile of society, currently, lead to a need for adaptations of health services to cope with various situations that have social, religious, political and economic characteristics as their causes. Chronic health conditions, for example, present a major challenge to the health sector, as there is evidence of a prevalent growth in its reality, thus requiring also structural empowerment to enable access and activities that involve this reality. , the research aimed to evaluate the institutional capacity to face chronic health conditions, in the Family Health Strategy of the municipality of Sobral - CE, from the perspective of Systemic Arterial Hypertension and Diabetes Mellitus. It is an evaluation study in the perspective of quantitative –qualitative approaches. It was anchored in the principles of De Salazar, using the Model for evaluating the effectiveness of interventions to control and prevent chronic noncommunicable diseases. The research field was the municipality Sobral, more specifically in the family health strategy of the headquarters. The subjects were the nurses and managers of each family health team, in which a questionnaire was applied to assess the institutional capacity to care for chronic conditions. Data analysis was performed using calculations to obtain the average by parts and the final average of all teams. Bardin content analysis was also used with the support of the N Vivo 11 software. The study was in accordance with the bioethical principles according to Resolution 466/12, of the National Health Council. favorable opinion from the Research Ethics Committee of the Universidade Estadual Vale do Acaraú, under the number: 2,054,329. The study showed that, in the understanding of nurses and managers, the municipality presented a reasonable capacity for attention to chronic conditions, with a final average of 7.61. In the same idea, the evaluation by the municipality's family health strategy follows, in which it was also a reasonable capacity with an average of 7.4 was classified. Another relevant aspect identified by the interviewees was due to the commitment of health management in the effectiveness of the municipality's health structure, making investments that were applied for the continuous development of the health of its users. Therefore, the maintenance of a practical execution and investments in the established municipal policy, will provide a constant and healthy growth to health, understanding it as a primordial organism that needs care for the correct and faithful functioning.
