Resumo: The tuberculosis is still one of the main priorities in public health because it is one disease determined by cultural and social economic of the population and it gets wose by the association of tuberculosis with AIDS. In Fortaleza the incidence present in the last few years as also the problem around the no adherence of therapy are the main reasons of this worry. This research aimed to analyse the factors that determined the no adherence of tuberculosis treatment by the patients in the Health Center Oliveira Pombo - HCOP, in Fortaleza from 1995 to 1996. We use two methodological approaches; the quantitative to develop a descriptive study about an epidemiological situation of tuberculosis in Fortaleza and to clarify the reasons of no adherence to treatment in HCOP; the other qualitative with the aim to understand the problem we are studying. So we interview fifteen patients who had on adherence to treatment and which evidences permitus to analyse the knowledge and the perception of themselves related to the disease and treatment. In short, we conclude that tuberculosis brings serious individual and social consequences due to the stigma as the disease is seen not only by the society but, by the own patient ; the comprehension of the disease and treatment was understood through the changes in behaviour and expression of feelings caused by the impact of diagnosis . The causes reasons of this no adherence were pointed out as one personal decision, however we think that the health service was a partner ship in this process. In this point of view in order to minimize the problem, we build a proposal of measures to be used not only by the HCOP but that can be used at any other unity of health.
