Several adhesive strategies have emerged in order to improve performance of self-etch adhesives system, but to get an effective long-term adhesion in dentin substrate remains a challenge for these systems. It was recently suggested the use of EDTA as a dentin conditioning, which seems to have considerably improved the bond strength in laboratory tests. However, the results of laboratory studies cannot always predict the clinical behavior of materials for more complexes and elaborate that they can be. In this sense, it identifies the importance of a longitudinal clinical trial to confirm the effectiveness of this dentin conditioning associated to a self-etch adhesive. The aim of this study was to evaluate the clinical effectiveness of conditioning with EDTA, in adhesion with a self-etch adhesive system in non-carious cervical lesions [LCNC] for a period of up to 6 months. Ninety-six LCNC with features of scleroses were selected within forty-eight patients according to inclusion criteria. Each patient must have two non-carious cervical lesions with similar size, expelled and at least 50 of margins without enamel, with the neck located in dentin / cementum. Three calibrated operators unaware of the randomization procedures for the placement of restorations and conducting clinical evaluations in the time immediately and 6 months after placement of restorations. The criteria used for evaluation were the Federation Dental International (FDI): fracture / retention, marginal adaptation, postoperative sensitivity, and caries adjacent to restoration. Data were analyzed by distribution descriptive analyze, Fisher´s exact and Mc Nemar´s tests. Sensitivity was observed preoperatively in 63.5 of spontaneous LCNC, but in the immediate postoperative period no patient had a sensitivity, and evaluation of 6 months only 2.1. Just a restoration released, and another presented with a fracture of the margins to the time of evaluation, both in the experimental group without EDTA, achieving a retention rate of 100 in the experimental group and 95.7 in the control group, but no statistically significant difference was detected (p 0.05). Both groups had similar rates of marginal desadaptation criterion "B" FDI at 6 months (p 0.05), however, both showed significant difference compared to baseline (p 0.05). The present study demonstrates excellent clinical performance of the adhesive system Adper Easy One with and without use of EDTA after 6 months, and this adhesive system would have the provisional acceptance of the ADA. However assessments with longer longevity need to be performed. |
