Resumo: Cardiopulmonary fitness assessment is a valuable resource to obtain quantitative indicators of an individual's physical performance. The cardiopulmonary exercise test (CPX), considered the gold standard test for this evaluation, is costly and difficult to be accessed for the general population. In order to make this evaluation more accessible, minimizing costs, and to better reflect the performance of daily life activities, alternative tests were proposed. Morbidly obese patients present limitations that limit physical performance assessment through CPX and could benefit from a test of shorter duration, provided it is validated. Aim: To validate the two-minute step test (2MST) in morbidly obese patients with or without associated comorbidities, in relation to the CPX. Methods: Observational study including 31 obese individuals (BMI>35kg/m²). The CPX and the 2MST were performed and metabolic and ventilatory parameters were recorded. Pearson correlation analysis and multiple linear regression analysis were performed to evaluate the CPX peak VO2 best predictors. Bland-Altman analysis was performed to quantify the agreement between the two methods. Results: CPX peak VO2 and 2MST peak VO2 were significantly correlated (r=0.70, p<0.001); the 2MST number of step cycles (UDS) was correlated with the distance covered during the CPX (r=0.39, p=0.03) and with the CPX peak VO2 (r=0.38, p=0.03). The Bland-Altman analysis showed satisfactory agreement between the methods. The reference equation was obtained: VO2(mL.kg-1.min-1)=21.888+(0.0874*total UDS)-(0.206*BMI)-(0.053*Age), with estimated standard error=1.75mL.kg-1.min-1. Conclusion: The 2MST is a viable, practical and easily accessible test for cardiopulmonary fitness evaluation. UDS, BMI and age can satisfactorily predict peak VO2.
