Resumo: Population aging is a major corporate transformation and has represented the subject of numerous studies. With the increase in life expectancy increases the demand for more common in this age of health services due to chronic disease. Also it appears the realization of a significant number of hospitalizations, and is considered an important element of support for dependent elderly network. During hospitalization are experienced diverse experiences that culminate in the decision making with respect to the post-hospital period. Participate in this organization for the elderly and families process, as the professional social worker. Regarding the particularities of professional practice with older adults in hospitals, highlights the existence of a limited production of knowledge on the subject and at the same time, professional challenges as the liaison with the social-support network. Accordingly, this study sought to identify aspects involving home care as an alternative to frequent readmissions. The study showed a highly qualitative and set out to unravel the meanings of hospitalization for seniors and families, in addition to aspects involving the professional performance of the social worker and the prospects of home care through observation and interviews of health professionals.
