The medical student is exposed to intense emotional situations that can determine psychological distress and impairment in academic performance. There is a high suicide rate among doctors and medical students and the relevant scientific literature identifies the corroborative environmental factors but does not point to the main psychic trends. Thus, the objectives of this research were: to identify situations of adaptive ineffectiveness in medical students of the Faculty of Medical and Health Sciences (FCMS) of the Pontifical Catholic University of São Paulo (PUC-SP) by applying the Operational Adaptive Diagnostic Scale Instrument Self-report (AR) and to verify if there is an increase in the adaptive inefficacy of medical students in the different years of graduation. This is a predominantly quantitative cross-sectional and exploratory study with medical students from FCMS of PUC-SP, male and female, aged 18 years or older, from the first to sixth years, who agreed to answer the sociodemographic questionnaire and EDAO-AR and with the institutional psychologist of this same institution, through semi-directed interview, after approval by the Institutional Research Ethics Committee. The quantitative data obtained were statistically analyzed using the Stata™ software, version 15.1 and the open-ended questions were grouped according to the categorization proposed by Bardin. It was observed that having a history of mental illness in the family doubles the chances of the outcome of interest being observed, with 44.6% reporting a positive history. More than half of the students use at least one type of drug, which may be licit or illicit, with higher consumption among male students. Women were approximately twice as likely to be ineffective as compared to men in an unadjusted model. Four out of ten students have thought of dropping out of the course at some point, and among the most prevalent reasons reported were categorized into Difficulty of Adequacy and Negative Feelings. History of mental illness in the family, living with friends in relation to living alone and not having religious belief were factors independently associated with ineffective adequacy. It is noted that 11.9% of the sample was classified as Poorly Affective-relational aspect. This percentage was 6.1% in relation to the Productive aspect. In classification, 14.1% of the students presented Ineffective Adaptation, which can be graded as mild (10.2%) or moderate (3.9%). In order to verify the possible association between the course year and the criteria used to assess adequacy, it was concluded that the longer the course year, the lower the chance of ineffective adequacy, students in more advanced stages of medical school were more protected (adjusted model). Finally, the result of the data obtained through the analysis reveals the current mental health situation of the medical students of FCMS from PUC-SP |
