Resumo: |
The main objective of this dissertation is to discuss the work of the group of nurses involved in hospital discharge procedures in a public Health Unit, taking into account the nursing assistance in helping the elderly who suffered a stroke since this condition requires adequate care by the family and caretakers at home and it is crucial that the nursing professionals are aware of the rehabilitation procedures and information necessary to assisting and supervising the elderly patient. The assistant concerning cultural and biopsychosocial care contributing to the safety of the elderly, his independence and socialization can help him in interacting with the members of his family |
