Resumo: The dissertation focuses on the historicity of the conceptions of the health disease in societies as ideal constructions of concrete realities and practices developed by social workers in the Primary Health Care. General Purpose: Identify what this process is about among social workers working in UBS. Specific: Actions, job prospects, relationship between concepts and practice: contribute to the debate on professional practice in health. Theoretical framework: Concepts about the health disease process in Western societies; work category in the development of capitalism and services; health policy and attention to Primary Health Care in Brazil and São Paulo; insertion of Social Work in Health. Methodology: Literature search, documents, database of SMSSP, Basic Units of Health Health Technical Supervision of UBS-/Lapa- /Pinheiros, Central West Region. Semi-structured interviews with social workers of these UBSs: conceptions about the health-disease process, actions taken and work expectations. Results: UBS organized under different models: Programmatic Actions, Family Health Strategy, Mixed and Integral. Predominance of female professionals, two sub-groups between the age groups 20-39 years and 50-60 years. With UBS working experience between 1 and 10 years. With training time between 6 and 10 years in different educational institutions. More frequent specialization in Family, followed by Public Health. Different conceptions of the process as the WHO definition, the multifactorial and social determination. Several actions developed in practice. Conclusions: Conceptions of the health-disease process predominantly multifactorial, daily professional practice developed most often in precarious health facilities, predominantly aid-oriented actions
