AIMS: this article aims to analyze the main indications for blood transfusion and its main risks and effects in postoperative heart surgeriesSOURCE OF DATA: scientific original and review articles from the MedLine/Pubmed, Scielo and LILACS databases. SUMMARY OF FINDINGS: the indications for postoperative transfusion in cardiac surgery include treatment of coagulopathies and correction of anemia with the ultimate goal of limiting bleeding and improve the ability to transport oxygen. Various evidences suggest the association between blood transfusion and increased risk of postoperative complications in cardiac surgery. CONCLUSIONS: blood transfusion is associated with higher rates of comorbidities, such as infectious processes, atrial fibrillation, acute renal failure, stroke and ARDS.
