O efeito do ultrassom terapêutico no tratamento cirúrgico da síndrome do túnel do carpo: estudo piloto

Resumo: Objective: A randomized clinical trial was used to assess the efficacy of a physical therapy treatment protocol that employs pulsed-mode (20%) ultrasound at a frequency of 3 mhz for the rehabilitation of patients submitted to carpal tunnel syndrome surgical release. Metodology: The study sample comprised 15 patients and 18 hands, who were randomized into two groups: intervention group (ultrasound), and the control group. All patients were assessed preoperatively and after 20 sessions of physical therapy; the following parameters were considered: hand grip strength and pinch strength (using a JAMAR dynamometer), Semmes-Weisntein monofilament sensibility; visual analog scale; distal motor latency (milliseconds), and median nerve sensory conduction velocity (meters/second).Results: The coefficient of variation (VC) of grip and pinch strengths of ultrasound and control groups 22,8% and 28,3%, respectively. Hence, no statistically significant difference was found between both coefficients (p=0. 391). There was only a statiscally significant difference between groups regarding double pinch strength (p=0. 006). The ultrasound group showed significantly more double pinch strength when compared to the control group. Of remaining variables, the difference between groups was not statistically significant. Conclusion: The results of this study suggest that the therapeutic use of ultrasound with the employed parameters (3 MHz frequency, 0,8 W/cm intensity in pulse-mode 1:5 (20%), during 4 minutes) is effective when comparing between groups. There was no statistical significant difference among other variables when comparing both treatament methods.
