Resumo: There is a limited number of published studies regarding buccal fat pad removal for aesthetic contouring of the midface and lack of patient follow-up. The aim of this study is to describe the intraoral surgical protocol of the Bichectomy technique, with aesthetic and functional purposes and to show the relationship of the facial type with the amount of adipose tissue removed. The Bichectomy technique described here was applied systematically to 58 patients. Sex, age, face type and the amount of adipose tissue removed in this surgical protocol were evaluated. All data was submitted to statistical analysis with a significance level of 0.05. From the 58 patients, 89.5% were women. The median age was 35 (19-62) and 64% of the patients had a rounded face. The amount of adipose tissue removed was significantly greater in patients with a round face. There was no difference in the amount of removed tissue from left and right side. It was concluded that, if this protocol is followed correctly, Bichectomy surgery can be performed with precision, safety and predictability. The amount of tissue removed was equal on both sides and the total amount of adipose tissue removed was greater in the rounded facial types.
