O objetivo do estudo foi avaliar o envolvimento renal em pacientes portadores de LES, acompanhados no Serviço de Reumatologia do Departamento de Clínica Médica da Faculdade de Medicina da Universidade Federal de Goiás, determinar a freqüência de nefrite lúpica de acordo com critérios clínico-laboratoriais e histopatológicos, classificá-las de acordo com a classificação da OMS e ISN/RPS 2003, e tentar correlacionar os dados encontrados. Nefrite lúpica foi definida pela presença de proteinúria >200mg/24 horas e/ou hematúria (>8000 hemáceas/mL) associada a leucocitúria estéril e/ou cilindrúria. Insuficiência renal foi definida pela presença de creatinina sérica > 1,3 mg/dL e/ou depuração de creatinina < 60 mL/min/1,73 m² de superfície corpórea. Todos os pacientes com nefrite lúpica e/ou insuficiência renal foram submetidos à realização de biópsia renal. Os pacientes foram divididos em dois grupos: lúpus com nefrite e sem nefrite. O grupo com nefrite por sua vez foi dividido em dois subgrupos: nefrite com função renal normal e nefrite com insuficiência renal. A comparação de dados clínicos e laboratoriais entre os grupos com nefrite e sem nefrite, revelou maior prevalência de HAS, maior creatinina sérica, maior proteinúria, menor depuração de creatinina e maior comprometimento renal no momento do diagnóstico da doença no grupo com nefrite. A presença de insuficiência renal ocorreu somente no grupo com nefrite. Em pacientes do subgrupo nefrite com insuficiência renal foi observada maior creatinina sérica, menor depuração de creatinina indicando lesão renal mais grave neste subgrupo. FAN positivo foi mais freqüente nos pacientes do subgrupo nefrite lúpica com insuficiência renal. A classe IV foi o tipo histológico mais freqüente, observado tanto na classificação da OMS como na nova classificação da ISN/RPS 2003 sendo que nesta última classificação houve um predomínio da classe IV-G (global). A prevalência de insuficiência renal encontrada no estudo foi de 20% e entre os pacientes com nefrite foi 44%, sendo esses resultados preocupantes, pois a doença renal e a insuficiência renal interferem na qualidade de vida destes pacientes, e são freqüentes causas de óbito. ________________________________________________________________________________________ ABSTRACTRenal involvement in systemic lupus erithematosus (SLE) is a recurrent complication of this disease, besides being related to a greater morbidity and mortality. The study objective was to evaluate the renal involvement in patients with SLE, followed by the Rheumatology Service of the Medical Clinic Departament of the Faculty of the Federal University of Goiás Federal University, to determine the frequency of Lupus nephritis according to the clinicopathologic laboratory criteria, classifying them according to the classification of WHO and ISN/RPS 2003, and trying to correlate the data found. Lupus nephritis was defined by the presence of proteinuria >200mg/24 hours and/or hematuria (>8000 haemacies/mL) associated to esteril pyuria and/or cilindruria. Renal insufficiency was defined by the presence of serum creatinine>1,3mg/dl and/or depuration of creatinine<60mL/min/1,73m². All patients with Lupus nephritis and/or renal insufficiency were submitted to renal biopsy. The patients were divided in two groups: lupus with nephritis and without nephritis. The group with nephritis was divided in two subgroups: nephritis with normal renal function and nephritis with renal insufficiency. The comparison of some clinical and laboratorial data between the groups with nephritis and without nephritis, showed a greater prevalence of HAS, greater serum creatinine, greater proteinuria, less depuration of creatinine and greater renal involvement at the moment of the diagnosis of the disease in the group with nephritis. The presence of renal insufficiency occurred only in the group with nephritis. Patients from the nephritis with renal insufficiency subgroup was possible to see that they had greater serum creatinine, less depuration of creatinine indicating more serious renal injury in this subgroup. Positive AAN was more frequent in lupus nephritis subgroup.Class IV was the histological type of lupus nephritis more frequent, noticing both the WHO classification and the ISN/RPS 2003 new classification. According with the new classification, class IV-G was predominant.
