Perfil clínico e epidemiológico dos portadores de mielorradiculopatia esquistossomótica em duas unidades de saúde de Aracaju, Sergipe

Summary: INTRODUCTION: Schistosomal myeloradiculopathy (SMR) is the most severe and disabling ectopic infection of Schistosoma mansoni, however its prevalence in endemic areas has been underestimated over the years. The aim of this study was to identify clinical and epidemiological data related to cases of MRE diagnosed in two health units. METHODS: A retrospective, descriptive case series from the data collection records for the years 2007-2011, in two hospitals of reference in Sergipe. RESULTS: We identified 27 patients in which 16 (59.25%) were male with a mean age of 32.55 years and a standard deviation of 13.47. Only 51.8% of subjects were asked whether area of residence or visitation was rural and 55.5% of patients were asked if the region was endemic for schistosomiasis. Signs and symptoms more common were back pain (62,9%) and urinary retention (74%) . Six (22.2%) patients underwent examination of cerebrospinal fluid, only 12 (44.4%) underwent stool testing and in three (11.1%) the result was positive for S. mansoni. Only in 13 cases was performed nuclear magnetic resonance. The lumbar segments (61,5%) and thoracolumbar (23,07%) of the spinal cord were the areas most affected, followed by the thoracic (15,38%). CONCLUSION: Analyzed the medical records revealed failure in data collection during important clinical and epidemiological anamnesis which contributes to the underdiagnosis of SRM in Sergipe. Knowledge of clinical and epidemiological profile of this ectopic could provide political control and prevention more effective and thereby reduce morbidity and mortality of the population.

O objetivo deste estudo foi identificar os dados clínicos e epidemiológicos referentes aos casos de MRE diagnosticados em duas unidades de saúde. MÉTODO: Estudo retrospectivo, descritivo do tipo série de casos a partir da coleta de dados em prontuários, referentes aos anos de 2007 a 2011, em dois hospitais de referência de Sergipe. RESULTADOS: Foram identificados 27 pacientes onde 16 (59,25%) eram do gênero masculino com idade média de 32,55 anos e desvio padrão de 13,47. Apenas 51,8% dos indivíduos foram questionados se a área de residência ou visitação era área rural e 55,5% dos pacientes foram perguntados se a região era endêmica para esquistossomose. Os sinais/sintomas mais comuns foram retenção urinária (74 %) e dor lombar (62,9%), Seis (22,2%) pacientes foram submetidos ao exame de líquor, apenas 12 (44,4%) realizaram exame parasitológico de fezes e em três (11,1%) o resultado foi positivo para S. mansoni. Somente em 13 casos foi realizada ressonância magnética. Os segmentos lombar (61,5%) e tóraco-lombar (23,07%) da medula espinhal foram as áreas mais acometidas, seguidas pelo segmento torácico (15,38%). CONCLUSÃO: Os registros médicos analisados revelaram falha na coleta de dados clínicos e epidemiológicos importantes durante a anamnese o que contribui para o subdiagnóstico da MRE em Sergipe.
